Communicates with Physician using Situation, Background, Assessment and Recommendation format. Contributes to the interdisciplinary team to manage Wound Care patients. Patient documentation is accurate and timely and it reflects rationale for any deviation from standards of care. Practice is congruent with evidence based wound care guidelines. Observing Universal Protocol at all times provides clinical assistance to patients and notifies appropriate medical personnel of potential critical patient conditions. Ensures coordination and continuity of care with community and medical resources as indicated. Overview Associate or bachelor's degree in nursing. Position Summary The Nurse Practitioner provides high quality, patient-focused health care services to all patients in collaboration with the other staff members to ensure that the individually planned needs and desires of patients are met, regardless of age or gender, on a continuing basis by performing the duties outlined below. All entries into the medical record whether electronic or on paper will be legible, complete, dated, timed and signed with appropriate credentials. Documentation will be completed prior to discharge from the facility when possible but no later than 48 hours after discharge from the facticity. Utilizes all equipment, supplies, facilities, and resources of the Company in a prudent and efficient manner. Participates in department quality improvement activities, staff educational programs, attends departmental meetings, and demonstrates and maintains clinical competency in the delivery of patient care services in accordance with Company guidelines. Participates in Peer Review and Chart Audit processes as designated by the Medical Director. Maintains supervisory duties over staff as required. Must maintain collaboration with the physician to deal with issues outside of their scope of practice. Maintains competency in skills relevant to the scope of services provided in the facility. The scope of services in specific markets will require employees to complete relevant training and to obtain additional certifications or licensure. (DOT Certification, Occupational Medicine Certification, Dispensing Permits, Telemedicine permits, etc.) Other duties as needed.
