Swan Hospice is looking for CNA( Certified Nursing Assistants ) with Hospice experience to join our team.

We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities and in private homes. We treat residents, families and loved ones with kindness, respect, compassion and dignity. We have a comprehensive team that can provide support and stability to residents and staff.

Job Responsibilities

  • Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.

  • Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.

  • Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.

  • Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

  • Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.

  • Feeds and assists with meals.

  • Obtains temperature, pulse and respiration as supervised by the Hospice Nurse.

  • Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information , including the date and time of the report and the person receiving the information.

Ideal candidates will have:

  • CNA for Michigan

  • Understands the hospice philosophy

  • Minimum of one year experience working in hospice

  • Dependable transportation

State of Michigan - Certified Nursing Assistant Full Time Night Shift (6:30pm - 7:00am) Position Summary: Under the direction of a Licensed Nurse, supports the Nursing Staff by performing basic nursing care to the resident. Assist and maintains a safe and clean work environment. Maintains resident privacy, dignity, confidentiality, and respects resident’s rights. Essential Functions and Responsibilities: Hourly Rounding including: Pain, Position, Personal needs, and Possessions Assist residents in and out of bed and into a wheelchair using the correct lifting and transferring technique. Gives residents baths. Assists residents in dressing and undressing. Assists residents in using the bathroom and giving personal care. Assists residents to dining area for meals. Helps feed residents. Strips beds and replaces with clean linens. Turn the resident intermittently if bedridden due to illness. Assist with toileting via bedpan, urinal, bedside commode, or to the bathroom, following toileting schedules. Document information regarding resident’s health. Clean residents’ closets and bedside cabinets. Transfer residents according to prescribed transfer technique. Rinse soiled linen/handle soiled linen in accordance with Infection Control Policy/Standard Precautions. Position residents in bed, using a lift or manually, lifting from chair to bed. Assist in and out of shower and routinely assist resident in range of motion exercise. Follow turning and OOB schedules. Bend, twist and stoop on a regular basis and be able to lift a minimum of 75 pounds. Assist residents with exercises, walking and encourage self help progress. Wash bed pans, urinals, commodes and wheelchairs. Pass feeding trays and assist residents in eating and picking up trays. Pass juice and snacks as required. Continuously make resident care rounds, placing side rails, reposition residents, encouraging and alleviating distress using proper methodology. Change and clean up an incontinent resident. Answer all resident calls and lights not just those on your assignment. Assist residents with activities during shift.
