Job Responsibilities Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient. Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry. Feeds and assists with meals. Obtains temperature, pulse and respiration as supervised by the Hospice Nurse. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information , including the date and time of the report and the person receiving the information. RN Chart Auditor The duties of an RN chart auditor are to review medical records for accuracy, quality assurance, or billing and insurance purposes. The responsibilities include assessing medical records to decide whether there is a valid reason for treatment or denial of care and ensure that the quality of patient care abides by health care policies and standards. Primary Responsibilities Review and audit medical records: examine patient charts to ensure completeness, accuracy and adherence to legal, regulatory, and institutional standards, including HIPAA, Medicare and Medicaid guidelines. Documentation improvement: provide feedback to healthcare staff, including physicians, nurses, home health aides to clarify or correct documentation issues. Reporting: prepare detailed audit reports highlighting deficiencies, trends, and recommendations for process improvements. Training and support: assist in training new hires on proper documentation and auditing procedures. Education and Experience: Education: Graduate of an accredited registered nursing program Bachelor’s degree in Nursing preferred. Certified Hospice and Palliative Nurse (CHPN) certification preferred. Experience: Has minimum of one (1) year hospice nursing experience. BASIC PURPOSE Oversees primary caseload of Hospice patients and provides skilled professional nursing care to them, their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, New Jersey Hospice licensure laws, accrediting body standards and agency policies and procedures. Provides the initial assessment and comprehensive nursing assessment and identification of problems of each patient; reassessment of nursing needs and collaborates to develop the Hospice Interdisciplinary Team (IDT) plan of care. Implements the plan of care and reevaluates and revises the plan of care relative to the patient’s nursing needs to provide appropriate, comprehensive and responsive goals and interventions. Teaches, supervises, and counsels the patient, family members, and staff regarding nursing care and the patient's needs, including other related problems of the patient at home Works closely with other members of the IDT to ensure all patient and family care needs are being met.
