Overview

VNS Health Community Health RN's serve as a member of an interdisciplinary care team within a NORC (Naturally Occurring Retirement Community) or senior living community, coordinating a broad range of health and social services to support older adults in aging safely in place.

NORCs achieve this by facilitating and integrating existing community-based health and social services, while identifying and organizing additional supports needed to help residents remain independent and engaged in their homes and communities.

What You Will Do:

  • Collaborates with interdisciplinary teams to coordinate health and wellness activities.
  • Conducts client assessments to determine health needs, including taking vital signs as needed.
  • Accurately documents assessments, interventions, and outcomes in clients’ medical records.
  • Provides emergency care until EMS arrives and liaises with EMS personnel during medical emergencies.
  • Develops and implement individualized nursing care plans.
  • Health Promotion: Educates clients on disease prevention and health maintenance.
  • Assists in preparing statistical reports for funding sources (e.g., number of health assessments and referrals to primary care or specialists).
  • Care Coordination and Advocacy: Facilitates referrals and linkages to appropriate healthcare services based on client needs.

Qualifications

Licenses and Certifications:

  • License and current registration to practice as a Registered Professional Nurse in New York State required
  • CPR/BLS Certification required

 

Education:

  • Associate's Degree Nursing required Bachelor's Degree Nursing preferred

Work Experience:

  •  Minimum two years of experience working as an RN 
  • Prior experience working with behavioral health client population and providing clinical assessments of their health needs preferre

Pay Range

USD $40.95 - USD $51.19 /Hr.

Also responsible for adhering to NFP national guidelines. • Assesses the physical, social and psychological needs and environmental conditions of the patient/client through professional knowledge, skills of observation, interviewing, and physical examination to develop a plan of care. • Understands, interprets, and implements reimbursement protocols; determines the billing source for patient/client care. Consults with management on complex/unique reimbursement cases. • Acts as the coordinator of care for the multidisciplinary team by providing and managing the care needs of patients/clients which includes making/confirming referrals to other disciplines/community agencies and ordering supplies/services and ensuring they are delivered as needed. • Documents content and process of all services provided in accordance with government regulations/requirements, and VNS Health policies, practices, and procedures, as well as NFP requirements as appropriate. Ensures that physician’s orders, discharge summary and communication to team members, patient and family members adhere to VNS Health and regulatory requirements. • Communicates and consults with patient’s MD and follows-up on problematic situations affecting care such as abnormal clinical findings, medication discrepancies, or other factors that impact patient’s ability to comply with the plan of care.
