Licensed Practical Nurse, Hospice Home Care, Manhattan

Overview Provides high quality nursing care and personal care to patients under the direction of the registered professional nurse consistent with VNS Health philosophy, policy, goals and objectives, and Standards of Nursing Practice. Provides nursing care to patients in any of the service areas. Works under moderate supervision. Deliver personalized nursing and care management by constantly evaluating the evolving patient needs and adjusting the plan of care. Carry out quality-driven and care coordination by communicating within the VNS Health interdisciplinary team and across providers and other resources to deliver the plan of care effectively and efficiently. Practice in the community, traveling to patients’ homes or care facilities, using effective time management and agility to manage weekly caseload and other duties. Use agility and empathy to deliver continuity of patient care including coordinating supplies and providing education. Use technology, including a tablet computer, phone, and other systems to coordinate and maintain accurate and up-to-date electronic medical records (EMRs) for patients in the community. Provide empowering education and support to patients and their families to help them understand and navigate end of life treatment. Qualifications Licenses and Certifications: License and current registration to practice as an LPN in New York State required Valid driver's license required for all areas outside of Manhattan Education: High School Diploma or completion of LPN Program required Work Experience: Minimum of one year work experience as an LPN preferred Home care/hospice experience preferred Employee will be hired to perform nursing care duties in primarily one area of service (Home Care visits, Hospice Residence shifts, or Crisis Care visits) However the nurse will be cross–trained in each service area and will be expected to perform nursing care duties in all three Hospice services, as needed Pay Range USD $38.23 - USD $40.00 /Hr. Home Health Aide Supervisor

Overview Supervises Home Health Aides delivering service to clients in the home care setting and coordinates all related administrative responsibilities. Works under general supervision. • Assigns Home Health Aides (HHA) to deliver requested service to VNS Health patients and members. • Ensures the delivery of high-quality service by supervising activities of assigned HHA staff. • Maintains compliance of HHAs with corporate policies, state regulations, and contractual requirements. • Initiates appropriate corrective action and documentation by coaching, counseling, and terminating HHAs as required, in a timely effective manner. • Encourages and develops on-going communication with HHA staff and management across VNS Health; identifies and resolves issues which effect service delivery. • Maintains or grows caseload depending on resource availability. • Develops and implements procedures to maximize operational efficiency and effectiveness, as necessary. • Performs all duties inherent in a supervisory role. Ensures effective staff training, interviews candidates for employment, evaluates staff performance and recommends hiring, promotions, salary actions and terminations, as appropriate. • Participates in special projects and performs other duties as assigned.
