Your Role:

  • Provide compassionate care with dignity and respect for residents

  • Monitor vital signs and report changes in resident behavior to the nursing and medical team

  • Assist residents with daily care needs while maintaining comfort and safety

  • Routinely make rounds and provide responsive resident care and services

  • Support resident independence and well-being through patient-centered care

  • Maintain accurate communication with nursing staff regarding resident needs

  • Ensure reliable attendance and punctuality for assigned shifts

  • Follow facility policies, procedures, and long-term care standards

  • Perform additional duties as assigned by management in a resident-centered care environment

Who We’re Looking For:

  • Current Illinois CNA certification required

  • Graduate of an accredited CNA program

  • Caring, dependable, and team-oriented attitude

This is an Hourly position. Overview POSITION SUMMARY: ​The Home Care Aide is under the direction of a Registered Nurse or Licensed Physical Therapist/Occupational Therapist and in accordance with a plan of treatment to provide personal care and emotional comfort to patients in a variety of settings. The Home Care aide travels to patient homes and provides in-home care to assigned patients daily. The care provided must be age appropriate and the physical, emotional, and psychosocial needs of the patients are to be met. Qualifications REQUIRED QUALIFICATIONS: Education: High School/GED Licenses/Certifications: CNA-Certified Nurses Assistant Other Licenses/Certifications: Valid Drivers License in current status Own transportation and auto insurance BLS Certification from the American Heart Association Other skills/knowledge: Solid computer skills, including proficiency with Microsoft software.
