1. Interview resident/families as necessary and make discharge arrangements. |
|
2. Maintain an excellent working relationship with the residents, family members, co-workers, etc. Monthly care plan meetings. |
|
3. Make oral reports/recommendations to the administrator concerning the operation of the social/- Works with Delta Dental, Vision, hospice, mental health etc. |
|
4. Report all incidents/accidents to the supervisor on duty. |
|
5. Provide information to resident/families as to the activity programs available to the residents. |
|
6. Be alert to any change in residents/condition. Report to charge/staff nurse immediately. |
|
7. Invite and encourage residents to participate in day’s activities and assist in discovering methods to motivate residents’ interest in life and the community. |
|
8. Work with the facility’s consultants as necessary and implement recommended changes as required. |
|
9. Make routine visits to residents and perform assistance with crafts, projects, etc., as necessary. |
|
10. Assist in arranging transportation for field trips, doctor appointments, etc. |
|
11. Help to create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the facility. |
|
12. Follow universal precautions. |
|
13. Maintain confidentiality of all resident information. |
