What You'll Do:

Provide companionship, conversation, and emotional support
Assist with activities of daily living: bathing, dressing, grooming, toileting, and oral hygiene
Support meal planning, preparation, and grocery shopping
Provide medication reminders and monitor client well-being
Maintain accurate documentation and timely reporting to the office
Adhere to SYNERGY HomeCare's infection control, HIPAA, and dress code policies
Report any changes in a client's physical, mental, or emotional condition promptly
Clock in and out accurately using our mobile app or assigned timekeeping method

Requirements:

High school diploma or GED equivalent
Valid driver's license and reliable transportation (preferred)
Proof of current 2-step TB testing (or ability to obtain)
Current CPR / First Aid certification (or ability to obtain)
Must be bondable and pass a national criminal background check
Satisfactory professional and personal reference check
Ability to complete SYNERGY HomeCare orientation and assigned online training modules
Physically able to perform light to moderate exertion including standing, walking, kneeling, and assisting with client repositioning

Garfield Ave. 2nd Floor, Alhambra, CA 91801 Compensation: $27.00 - $35.00 / hour Description We are seeking a compassionate, dedicated, and detail-oriented Certified Home Health Aide (CHHA) to join our hospice interdisciplinary team. This field-based role focuses on providing essential personal care and emotional support to terminally ill patients, ensuring comfort, dignity, and quality of life across the San Gabriel Valley (SGV) region. The ideal candidate will work independently in the field, confidently manage digital documentation, and possess a deep respect for diverse cultural backgrounds. Strong computer knowledge with previous hands-on experience using Electronic Medical Records (EMR) software, specifically HospiceMD, and comfort with digital expense tools like Concur. Valid driver’s license, a reliable and well-maintained vehicle, and active auto liability insurance to meet mandatory field travel requirements. Deep empathy for the ill and elderly, a calm demeanor around end-of-life care, high integrity, and strict adherence to HIPAA patient confidentiality regulations.
