The mission of PACE is to deliver person-centered care in the setting that’s most appropriate for each participant—whether that’s in their own home, with family, in long-term care, or assisted living.

PACE is a capitated model of care, which means there are no productivity metrics, only a focus on delivering the care each participant truly needs. Responsibilities

You Can Expect:

  • Monday – Friday 8:00am – 5:00pm – no weekends or holidays!
  • Dedicated team of providers, support staff, physical therapists, occupational therapists, dieticians, and social workers
  • 5 patients per day – small volume
  • Call is once every 7 weeks, 5pm – 8am, you are not expected to go out, calls are triaged before reaching you!

Required Qualifications

Experienced providers in palliative, hospice, primary care, or long-term care are encouraged to apply

Qualifications

Required Education: Graduate of an accredited Nursing Program, and Masters of Science Degree in Nursing (MSN)

Required Experience: Minimum of five (5+) years of progressive experience in area of specialization

Required Certifications and Licensures: Current licensure by the Virginia State Board of Nursing as a Nurse Practitioner with prescriptive authority. Maintains BLS and ACLS Certifications. Hold a current, active license as a registered nurse in Virginia or hold a current multistate licensure privilege as a registered nurse.
