Job Summary:

 

The Care Nurse Supervisor (CNS) manages the successful operation of the Resident Care Department, under the leadership of the Resident Care Director (RCD). The CNS is responsible for the oversight, coaching, and direct supervision of all resident care service providers in the community. The CNS management scope includes direct supervision of resident care positions, resident assessment, and management as defined by the Resident Care Director.

 

Principal Duties/Responsibilities:

 

  • Demonstrates expertise in clinical skills and nursing judgement within their scope of practice.
  • Identifies and assesses the clinical status of potential residents
  • Obtains the medical information release form from primary care physician as needed
  • Develops a plan of care (in collaboration with the RCD) based on the identified physical and psychological problems of the resident including: the identified problems; resident specific goals and planned program interventions. Successfully implements the plan of care
  • Develops a service plan (in collaboration with the RCD) according to the requirements outlined in the state regulations
  • Coordinates care needs with community providers via an effective case management process
  • Measures and accurately records vital signs (temperatures, pulse, respiration, blood pressure weight and height) where appropriate as per state regulations
  • Oversees ancillary health services as needed
  • Provides ongoing supervision of residents’ health related activities, complies with health care instructions explained by health care professionals; promptly and appropriately seeks and obtains medical attention when needed.
  • Recognizes and assists in the prevention of elder abuse, neglect, and exploitation (financial and other) and reports to appropriate sources per state regulations
  • Conducts SAMM / MAR and RCA documentation audits bi-monthly
  • Conducts assessments / reassessments per Benchmark standards
  • Assesses risk in a proactive manner
  • Facilitates weekly resident tracking meeting in absence of the RCD
  • Assures resident information is communicated to the staff through a report process and calendar i.e. appointments, outings, etc.
  • Communicates and documents residents’ material needs (i.e. briefs, clothing, toothpaste, pads, etc.)
  • Notifies RCD of incidents in a timely manner. Accurately completes incident reports prior to the end of the working day
  • Observes and reports any significant changes in resident behavior and health to the RCD, physician, family, RCAs, and Executive Director per state regulations
  • Responds to emergencies and personal emergency response system and knows when to call for backup

 

Compliance and Safety

  • Ensures compliance with all resident care related regulations (e.g. medication management)
  • Demonstrates infection prevention and control
  • Demonstrates personal and resident safety
  • Reviews and demonstrates fire, safety, and elopement procedures
  • Ensures that a visit occurs following an incident, accident, injury
  • Ensures physician and family notification per state regulations
  • Actively participates in the QA process per assignment

 

Direct Supervision

  • Directs and supervises the daily work assignments of the Resident Care Assistants (RCA). Serves as first point of contact for RCAs regarding assignment questions or concerns. Provides input into performance reviews and disciplinary actions as required.
  • Participates in the hiring and termination processes as they relate to resident care associates. Sources and screens new candidates and facilitate/co-facilitates terminations.
  • Conducts or supervises the scheduling of all RCA staff
  • Closely mentors, guides and directs the Lead Resident Care Assistants in all aspects of their job
  • Participates in training / in-services as required by Benchmark and state regulations

Education/Experience/Licensure/Certification:

  • Graduate of an approved L.P.N. program (per state requirements).
  • Has a valid, unrestricted nursing license.

  • Previous experience working with the elderly in a paid or volunteer position highly desirable.

May advise patient of preparation required for exam or additional testing and keeps patients informed of visit status, unforeseen delays and other relevant information pertaining to the patient’s visit Assists in resolving patient concerns by acting as an advocate, as appropriate Participates in practice and department meetings, trainings, etc. to promote continuous improvement and quality patient care Takes initiative to learn, understand, and stay current with community resources and the greater healthcare system while continuously participating in self-assessment of skills Maintains patient rights and abides by all confidentiality obligations pursuant to CCHC and HIPAA Provides clerical/office support as necessary (i.e. scheduling appointments, greeting and registering patients, scanning incoming medical records and paperwork, preparing medical records, answering phone calls and taking messages, faxing, etc.) Ability to float to other MACC practices when required Verifies and implements clinician’s orders for testing and/or procedures (EKG’s, dressing changes, spirometry, Holter monitors, etc.) Triages walk-in patients per established protocols Maintains communication and collaboration with interprofessional team and others to facilitate safe transitions and continuity of care delivery Qualifications Lpn (p015) - Education/Experience/Training Ability to read, write and communicate in English Graduate of an accredited Practical Nursing program Currently licensed as a Licensed Practical Nurse in the state of Massachusetts Basic Life Support Certification for Health Care Providers Strong interpersonal, customer service, time management skills Experience with current office technology and proficient with computer office applications (Electronic Medical/Health Records, MS Office, etc.) A Minimum of two years’ experience in a clinical setting preferred Rn (x21a) - Education/Experience/Training Ability to read, write and communicate in English Graduate of an accredited school of nursing Currently licensed as a Registered Nurse in the Commonwealth of Massachusetts Basic Life Support Certification for Health Care Providers Experience with current office technology and proficient with computer office applications (Electronic Medical/Health Records, MS Office, etc.) Minimum of two years’ experience in a clinical setting preferred Schedule Details: 16 Hours Per Week, Rotating Days, No Weekends and No holidays Organization : Medical Affiliates of Cape Cod Primary Location : Massachusetts-Orleans Department : MAC-Orleans Medical Practice Annual/Hourly: Hourly Hiring Pay Range: 30 - 35.29
