Schedule: Full-Time/Day Shift. 9a – 5:30p Mon – Fri or 9a – 7:30p 4 days a week between Mon – Friday. Weekend backup every 3 months.
Location: Covering Admissions in Lagrange, Hinsdale, Bolingbrook and Glen Oaks Hospitals and some community admissions in surrounding area.
- Travels to patients' locations, including resident sites and hospital rooms, to assess eligibility for Hospice care.
- Partners with hospital Case Management staff to meet patient and hospital staff needs and gain new referrals.
- Assesses the emotional condition of patients and families to determine readiness for Hospice and address questions and concerns.
- Coordinates patient treatment efforts and promote quality improvement and educational initiatives.
- Obtains required signatures and generate necessary electronic or paper documents for regulatory compliance and reimbursement.
- Assesses patients' physical condition, medical needs, current medications, and equipment requirements.
- Determines patients' primary physician, personal information, and special needs, and obtain necessary orders.
- Obtains approval from Hospice and primary care physicians to admit patients to the program and communicate care plan initiation.
- Updates Hospice Intake office and referral sources on the status of admissions and review referral documents prior to consultation visits.
- Communicates effectively with the Care Team, Home Medical Equipment, and Pharmacist regarding patient-related equipment and medications.
- Travels to hospital room or location of hospice patient for assessments, symptom management, and update plan of care as needed.
- Collaborates with hospital Case Management, Physicians, and Nursing Staff to facilitate discharge from the hospital to the appropriate level of care.

Knowledge, Skills, and Abilities:
- Demonstrates ability to communicate by reading, writing, and typing legibly, speaking and comprehending English effectively in order to carry out job requirements. [Required]
- Knowledge of nursing skills in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee. [Required]
- Demonstrates basic computer skills. [Required]
- Ability to read and interpret documents such as physician orders, plan of care documents, policies and procedures and other instructions furnished in written, oral, diagram, or schedule form. [Required]
- Ability to follow standard precaution guidelines for infection control, including when providing patient care. [Required]
- Ability to complete AdventHealth Hospice Care Orientation and Training, as necessary. [Required]
Education:
- Associate's of Nursing [Required]
- Bachelor's of Nursing [Preferred]
Field of Study:

Work Experience:
- 1+ years Registered Nurse in acute care nursing experience [Required]

Pay Range:
