Job Description:

We are actively hiring Home Health Caregivers (CNAs, HHAs, and PCAs) who are ready to start immediately. This role is ideal for dependable caregivers looking for fast placement, flexible scheduling, and ongoing home care opportunities.

You will support clients in their homes by assisting with daily living activities, personal care, and companionship.

Responsibilities:

  • Provide personal care assistance such as bathing, grooming, dressing, and hygiene support

  • Assist with mobility, transfers, and safe ambulation

  • Support meal preparation and feeding assistance when needed

  • Perform light housekeeping to maintain a safe and clean environment

  • Offer companionship and emotional support

  • Monitor and report any changes in client condition

  • Follow individualized care plans and caregiver instructions

Requirements:

  • CNA, HHA, or PCA certification/experience preferred

  • Prior experience in home care, assisted living, or similar setting

  • Reliable, responsible, and able to respond to short-notice shifts

  • Strong communication and attention to detail

  • Compassionate and patient nature

Implement appropriate interventions that can assist in supporting the youth and parent/guardian/caregiver. Provide member transportation as needed in connection with service delivery. Assist in the coordination of treatment planning and intensive family therapy. Provide coaching in support of decision making in both crisis and non-crisis situations. Attend necessary meetings, such as care planning, school, and hospital discharge meetings. Provide referrals and linkage to appropriate supports and services. Conduct member outreach as needed to engage members and families. Evenings and at least one weekend day is a requirement Qualifications Bachelor’s or associate’s degree from an accredited institution, counseling or related field required with one year minimum experience working with children/families and adolescents in child development, childhood mental illness, and family issues. Ability to communicate effectively verbally and in writing. Ability to develop and maintain positive working relationships with coworkers and providers. High energy level, superior interpersonal skills and ability to function in a team atmosphere. Must hold a valid driver's license, have access to an operational and insured vehicle and be willing to use it to transport clients. Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving culturally diverse populations.
