Position Summary
The Home Health LPN LVN SEIU - PAH is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders.
The Licensed Vocational Nurse provides skilled nursing care within the scope of the LVN/LPN Nurse Practice Act. Provides patient care in the home environment primarily to young, middle, and older adults.
Position Specific Responsibilities
- • Evaluates Home Health patient each visit for chief complaint, health care status, and other pertinent information, including cultural, psychosocial and spiritual considerations and communicates findings to MD, RN, Nursing Supervisor as appropriate.
• Provides patient/family/caretaker instruction regarding nursing care, medications, diet, treatments and appropriate instruction to optimize self-care and to ensure safety.
• Serves as patient advocate: listens to, evaluates and acts on complaints, needs, problems, and suggestions of patients, families, physicians and others. Provides patient/family/caregiver with DHS Complaint Hot Line number 100% of the time.
• Practices according to LVN Nurse Practice Act and utilizes the code for nurses established by the American Nurses’ Association as a guide for ethical decision making process.
• Continually evaluates the clients and family’s responses to intervention in order to determine progress toward goal attainment and to provide those services requiring substantial specialized nursing care.
• Initiates and maintains a liaison relationship with all appropriate health care providers to assure that all efforts effectively compliment one another. Assists with coordination of all services.
• Documentation/Information Management:
• All applicable paperwork will be turned in within 48 hours.
• Time card is correct, complete and signed when turned in every two weeks.
• Leadership/Supervision:
• As assigned, assists with orientation of new LVN staff.
• In conjunction with RN, manages and coordinate care for assigned patients with other disciplines as appropriate.
• Development:
• Assumes responsibility and accountability for own professional growth and practice. This includes maintaining current licensure and completing all required in-service education.
• Attends all assigned staff development activities for department including a minimum attendance of 75% at all scheduled work time staff meetings.
• Participates in/conducts in-service education, as requested.
• Performance Improvement:
• Participates as requested in agency Improving Organizational Performance activities.
• Promotes use of the agency C.A.R.E. methodology in problem solving.
• Budgetary:

• Meets productivity standards.
Education and/or Experience
- Required - Education sufficient for license.
- Required - 1 year Prior experienced as an LVN
- Preferred - 6 months Experience in home care.
Skills
- Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
- Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
- Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
- Other Skills and Abilities: Requires the ability to establish and maintain a courteous and cooperative (service-oriented) working relationship with patients, families and co-workers.
Certifications, Licenses, and Registrations
- Required upon hire: California Licensed Vocational Nurse (Vendor Managed)
- Required upon hire: National Provider BLS - American Heart Association (Vendor Managed)
This is a collective bargaining position:
Day Shift
5 days/week: Monday through Friday 8:00 am to 5:00 pm
2 Weekend day shifts, twice a month

Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. Routine use of standard medical equipment and tools associated with clinical care is essential.
