Further requirements include weekends every 5th weekend from 8:00am to 4:30pm and 2 holidays per year from 8:00am to 4:30pm. A caregiver who excels in this role will: Assess the appropriateness of the requested home care services and skilled needs identified under the guidance of a Registered Nurse. Coordinate initial and final orders on internal and external referrals for patients needing home care nursing, therapy and social work services. Incorporate clinical and financial knowledge with referral process and recommend appropriate alternative approaches when appropriate. Communicate the home care needs with scheduling, managers, reimbursement and other parties involved in the home care services process. Serve as the CCHCS information specialist.
