We are seeking a highly skilled and compassionate LPN Licensed Practical Nurse Case Manager to join our team on a full-time basis, working Monday through Friday.

About the LPN Licensed Practical Nurse Case Manager Role:

As an LPN Licensed Practical Nurse Case Manager at Genesee Senior Living, you will play a vital role in providing high-quality, patient-centered care to our residents. Your responsibilities will include:

  • Assessing and developing individualized care plans for residents in our assisted living community

  • Coordinating with healthcare professionals to ensure seamless transitions and optimal care outcomes

  • Providing direct patient care, including medication administration, wound management, and vital sign tracking

  • Maintaining accurate and up-to-date medical records and reports

  • Participating in quality improvement initiatives to drive excellence in patient care

  • Communicating effectively with residents, families, and healthcare teams to promote collaboration and care coordination

LPN Licensed Practical Nurse Case Manager Requirements:

  • Current LPN licensure in the state of New York

  • Minimum 1 year of experience as an LPN in a healthcare setting

  • Strong assessment and communication skills

  • Ability to work effectively in a fast-paced environment

  • Knowledge of patient care standards and regulations
