How you make a difference
The Discharge Planner Registered Nurse is responsible for conducting comprehensive patient assessments to ensure effective discharge planning and coordination of care. This role includes patient education, arranging follow-up services, and ensuring compliance with nursing standards and regulations. By facilitating a smooth transition, the Discharge Planner RN enhances patient satisfaction and improves health outcomes.
Key Responsibilities
- Clinical Review & Documentation: Conduct clinical reviews of off-site observations, outpatient, and acute care, ensuring accurate documentation in the COREMR System per department guidelines.
- Care Coordination & Management: Collaborate with Regional and Site Medical Directors, providers, and operations personnel to coordinate care, manage medical costs, and ensure timely accruals and statistics.
- Discharge Planning & Patient Education: Assess patient needs, develop individualized care plans, educate patients on post-discharge care, review medications, and coordinate follow-up appointments with LVNs.
- Regulatory Compliance & Quality Improvement: Maintain knowledge of corporate policies, ensure compliance with regulations, participate in quality improvement initiatives, and advocate for medical necessity in community programs.
- Communication & Collaboration: Establish positive relationships with hospital departments, community agencies, institutional staff, and re-entry services to facilitate effective transitions and continuity of care.

Education
- BSN from an accredited institution
Experience
- Two (2) years’ clinical experience with Discharge Coordination experience, preferably in acute care settings or correctional settings.
- Previous correctional/detention facility experience
Licenses/Certifications
- Unrestricted, current RN license in state of employment
- Must maintain all certifications, educational requirements, licensing, etc. for RN license
- Certification in case management or discharge planning, is a plus
