About the Role

Serenity Hospice - Penn is growing and seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join our dedicated team serving patients and families in Chester County, PA.

In this rewarding role, you will manage a caseload of hospice patients in their homes, assisted living facilities, or skilled nursing facilities, providing expert clinical care, symptom management, education, and emotional support while coordinating the interdisciplinary team to ensure the highest quality of end-of-life care.

Key Responsibilities:

  • Perform initial and ongoing assessments of patients and families to determine hospice needs

  • Develop, implement, and update individualized plans of care

  • Provide skilled nursing care, pain and symptom management, and medication administration

  • Educate patients and caregivers on disease process, self-care techniques, and end-of-life planning

  • Coordinate care with physicians, social workers, chaplains, aides, and volunteers

  • Document accurately and timely in electronic medical records

  • Participate in interdisciplinary team meetings

Qualifications

  • Current Pennsylvania RN license in good standing

  • Minimum of 1–2 years of nursing experience; hospice, home health, or med/surg experience strongly preferred

  • Strong clinical assessment and critical thinking skills

  • Excellent communication and interpersonal abilities

  • Compassionate, patient-focused approach with a commitment to dignity and comfort

  • Reliable transportation, valid driver’s license, and auto insurance (mileage reimbursed at IRS rate)

  • Ability to work independently while collaborating with a supportive team
