Job Summary

Under general supervision, the ambulatory care services (ACS) registered nurse (RN) provides patient care and education in an outpatient setting to diverse patients, families, and community members throughout the lifespan. The professional RN adheres to the American Nurses Association (ANA) standards of professional nursing and performance, and the ANA Code of Ethics to provide care to all healthcare consumers. Applies critical thinking, evidence-based practices and competence to collaborate with the health care team to promote excellence in healthcare consumer safety and quality outcomes. Utilizes the Theory of Human Caring and Caritas in daily practice. "Jean Watson, PhD, RN, AHN-BC, FAAN, Founder / Director, Watson Caring Science Institute."

Essential Functions:

  • Implement evidence-based plan with patient in coordination with healthcare team. Develop, modify patient care and treatment plans as indicated by patient’s responses and conditions. Provide safe, appropriate nursing care and patient education in an ambulatory clinic setting. Including but not limited to newborn visits, foot care, wound care, obstetric care, diabetes education, preoperative teaching. Safely administer medications and treatments as ordered, according to policy. (20%)
  • Nursing Assessment – Collects comprehensive data pertinent to the patient situation. Assess the needs of individuals, families and/or communities to identify potential health or safety problems. Communicates findings to patient and appropriate healthcare team members. Data maybe collected as the result of a face to face, video, or telephone encounter. (15%)
  • Partners with patient and interprofessional healthcare team to review assessment and plan measurable goals and outcomes and strategies for achievement. Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care. Advocates for patients and family in accordance with ANA code ethics and professional practice standards. (15%)
  • Support interdisciplinary healthcare team through supervision, education, and training of other clinic team members including medical assistants, educators and care navigators. The ACS RN is key participant in the health care team providing problem solving, escalation of patient care issues, outreach to patients and families, and coordination of services and care activities within the clinic setting and the community (15%)
  • Maintain accurate, detailed reports and records. (10%)
  • Prepare patients for, and assist with examinations and treatments (10%)
  • Nursing Diagnosis analyze assessment data to determine nursing diagnosis. (5%)
  • Direct and supervise less skilled nursing/health care personnel (5%)
  • Promote positive patient experience (5%)

Education:

  • Associate's Degree Required

Work Experience:

  • 1-3 years Experience in a direct patient care setting

Licenses:

  • BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association Required and
  • RN-Registered Nurse - DORA - Department of Regulatory Agencies Required and

Identifies missing equipment and/or supplies and makes arrangements to acquire those supplies. o Applies knowledge of operative procedure and physician preferences by anticipating physicians' needs. o Observant of overt changes in patient's condition that indicates a problem has emerged. o Evaluates and updates case information, i.e., preference cards, broken or misplaced equipment, back-ordered supplies, and recommended changes in procedure. o Remains aware of the proper use of equipment and instruments, and discourages improper use and handling by co-workers & physicians. o Is observant of the changing needs of the operative field and the surgical team, anticipates potential needs, and takes appropriate actions. o Acts as the specialist and resource for circulators who may be unfamiliar with positioning devices, specialty equipment, and/or implants needed. o Assigned and responsible for after-hour call on a rotational basis. o Ability to perform scrub duties in multiple services; ie: neuro and ortho STII able to perform spinal fusions in bother services. For HDU Department Only: Additional on call requirements above and beyond regular scheduled hours For Bronson Lakeview Hospital and Bronson South Haven Hospital: The RN is responsible for completing the Workday CBL –“Swing beds”. This is an annual CBL that describes the difference in a swing bed patient versus an in-patient or observation patient. The RN will understand the documentation differences and the specific care required for each individual swing bed patient. Bronson South Haven Hospital and Bronson Lakeview Hospital employees who are approved to work in a Swing Bed Facility only: Employees may be expected to cover shifts at Bronson Lakeview Hospital and/or Bronson South Haven Hospital depending on patient acuity and staffing needs. Virtual Registered Nurse (VRN) Role: The virtual registered nurse leverages technology to deliver patient care remotely, collaborating with bedside staff and the broader care team to ensure quality outcomes and timely discharge. Core responsibilities include, but not limited to: Conducting admission screening, patient/family education, discharge planning, and medication dual acknowledgments via secure digital platforms. Completing diagnostic testing questionnaires and supporting patient safety initiatives. Mentoring and supporting nursing staff and facilitating patient progression toward discharge using electronic health record (EHR) tools such as checklists and clinical pathways. Analyzing assessment data to identify patient problems, confirming issues with patients/families as appropriate, and documenting individualized care plans in collaboration with bedside RNs. Systematically evaluating the quality and effectiveness of virtual nursing services, ensuring compliance with professional standards and organizational policies.
