Advanced Nurse Practitioner needed for Visiting Physician Practice

PRIMARY RESPONSIBILITIES:

Initiate and complete detailed patient assessment & physical exams

Wellness exams and sick visits

Evaluate and manage common and acute chronic diseases

Order, interpret and evaluate diagnostic and lab results

Refer and collaborate with other providers

Prescribing pharmacological and non-pharmacological therapies

REQUIREMENTS:

Computer literate & EMR Proficiency

Unrestricted APN license in the State of NJ with at least 1 year clinical experience

Current DEA/CDS

Willingness and ability to work independently

Personable, passionate and energetic

Ability to travel within the region

Previous experience with Medicare eligible patients a plus.

Bi-lingual a plus

Unit Overview We are seeking a dedicated and compassionate Family Practice Provider to join our Cooper Care Alliance team. The ideal candidate will provide comprehensive, patient-centered care to individuals and families across all ages, focusing on prevention, wellness, and the management of chronic and acute conditions in an outpatient setting. Short Description The Advanced Practice Nurse assumes responsibility for advanced nursing practice within a clinical specialty. In consultation with other members of the health care team, strengthens nursing practice and promotes positive patient outcomes. Responsibilities include clinical practice, staff and patient education, and research. The Advanced Practice Nurse serves as a significant member of the multidisciplinary health care team in the inpatient, outpatient and community settings. Key Responsibilities: Provide high-quality, evidence-based primary care to a diverse patient population. Conduct physical exams, diagnose and treat illnesses, order and interpret diagnostic tests. Develop and manage treatment plans, including prescribing medications and coordinating referrals. Promote preventive care and health education to patients and families.
