About the Role:

As a State Tested Nursing Assistant at Concord Care Center of Sandusky, you will be responsible for providing direct care to our residents, including assistance with daily living activities, vital signs, and basic medical procedures. You will work under the supervision of our experienced nursing staff to ensure the highest level of care and comfort for our residents. Responsibilities:

• Provide direct care to residents, including assistance with daily living activities, vital signs, and basic medical procedures
• Work collaboratively with the interdisciplinary care team to develop and implement personalized care plans
• Maintain accurate and up-to-date records of resident care and observations
• Assist with transferring, lifting, and moving residents as needed
• Participate in quality improvement initiatives to enhance the overall care experience for our residents
• Provide emotional support and comfort to residents and their families
