Work Schedule

  • Full-Time (72 hours bi-weekly)
  • Shift Varies - Four 10 Hour Shifts, 2 Opening Shifts (6a - 4:30pm), & 2 Closing Shifts Closing (9am - 7:30pm)
  • On Call commitment - 1 day during the week and have an on-call weekend rotation of every 4th or 5th weekend.

Job Requirements

  • Associate's Degree or Diploma in Nursing
  • New hires required to obtain BSN within 5 years of hire.
  • Basic Life Support for Healthcare Providers (BLS) ACLS/PALS may be required but is dependent on unit or department
  • Registered Nurse
  • Knowledge, judgment, and skills derived from the principles of biological, physical, behavioral, social, and nursing sciences to meet complex health care needs at various stages of the life cycle
  • Up to 1 year Clinical in Nursing experience

Job Overview

This position applies the nursing process in providing direct and indirect holistic care to patients and their families. The position also collaborates, in a collegial manner, with physicians and other health care team members in meeting the patient’s needs. This position may delegate certain aspects of care to those under their direct supervision, within the scope of nursing practice.

Job Responsibilities

  • Demonstrates organizational responsibilities. Identifies areas for self-improvement through self-assessment and peer review. Functions in roles and provides leadership on the unit as needed and requested. Cooperates with instructor to facilitate learning experiences for students. Demonstrates knowledge and is supportive of organizational and unit changes. Supports organizational activities by participating in activities such as committees, staff meetings, councils. Utilizes chain of command appropriately. Builds collegial relationships with physicians and other health team members. Demonstrates autonomy in identifying opportunities for process improvement. Maintains and improves current.
  • Evaluates the plan of care for patient based on optimal specific patient outcomes. Documents the patient/family response to care including teaching. Collaborates with the patient/family and with other members of the health care team, including physicians, to revise plan of care as needed. Supervises the care that was delegated to other health care team members.
  • Implements plan of care for patient. Provides a safe and therapeutic environment while maintaining patient’s autonomy, dignity, and rights. Determines plan of care based on ethical principles as outlined in the TriHealth Code of Ethical Business and Professional Behavior. Acts as a patient advocate. Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity. Seeks available resources to help formulate ethical decisions. Prioritizes the workload according to patient’s needs, requests, and the needs of the unit/facility. Recognizes emergency situations, notifies appropriate personnel, and begins emergency measures.
  • Performs initial and ongoing assessment of patient and family. Completes initial assessment tool. Documents ongoing assessment per unit/TriHealth guidelines. Communicates assessment findings to other health care providers as appropriate. Includes health counseling and health teaching needs in assessment. Performs technical skills according to policy and procedure and accepted standards within their area of practice. Safely administers medications/treatments and monitors their effects. Use all equipment in a safe, appropriate manner.
  • Plans care for patients and family based on assessment, standards of care, and optimal specific outcomes. Initiates and individualizes appropriate patient care guidelines/plan of care or clinical pathways. Updates current plan of care as needed based on patient/family input and healthcare needs. Develops both short- and long-term goals with patient/family and healthcare team including discharge planning. Communicates plan of care to others.

MAJOR AREAS OF RESPONSIBILITY: Plan of Care: Observe and monitor patient conditions and perform OASIS assessments (If you don’t already know this assessment, we will teach you!) to develop an individualized care plan and adjust as needs change. Minimize Patient Risk: Administer medication as prescribed by the physician and help decrease re-hospitalizations by prioritizing visits for high-risk patients. Supervision: Oversee and supervise total care of patient provided by nurse aides and LPNs Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges. Family Educator/Advocate: Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding. Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaborate with the interdisciplinary team and physicians to ensure optimal care is provided. Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors. Operations: Attend in-service training and mandatory agency meetings HARD & SOFT SKILLS: Compassionate communicator with a positive attitude Patience is a virtue when working with patients, families, physicians, and coworkers Attention to detail is critical, as is being observant and following directions REQUIREMENTS: Registered Nurse with current license in the state of employment Minimum one-year experience as an RN in an acute care setting.
