Clinic Nurse I, Primary Care, Full-Time, Days

Join a world-class academic healthcare system, UChicago Medicine, as a Clinic Nurse (RN) in the Primary Care clinic at our Hyde Park and Cottage Grove location. The Clinic Nurse in the Cottage Grove Clinic will act as our Population Health nurse, with a large portion of the job duties related to project improvement and not related to direct patient care.

As a Clinic Nurse, you will provide patient services in the clinical and research area. You will perform a wide variety of nursing care services to patients under the direction of the Clinical Nursing Supervisor, Physician Director of the practice, and/or Director of Outpatient Operations. Such services can be therapeutic or diagnostic procedures and/or administration of medication. You may participate in all aspects of research clinical trial administration, including: screening, recruitment and interviewing of subject, collection of data from hospital records, telephone follow-up with subjects, and some data entry.

Essential Job Functions:

Focuses on Healthy Planet tasks (non-clinic)
• Spirometry follow up: reaching out to schedule, curbside or indoor (phone calls/mychart) (~500 comer patients)
• MMR patients (comer peds) outreach, pend orders/adding to nurse schedule
• Lead project: abnormal results need consistent follow up; reaching out to families based on level of abnormal, keeping appointment, interpreting new levels, possible 2nd follow up
Additional focus for cross coverage for medical advice calls and triage back up support
Focuses on project improvements, metrics, and patient outcomes, as directed by the Medical Director
Performs any duties involving direct patient care as assigned by the Clinical Nursing Manager nurse and directed by Nurse Practice Act and Hospital and Patient Services Sector policies and procedures, as needed
Observes and records vital signs, assesses patient reactions and reports changes to Licensed Medical Physician
Assists the physician with patient examinations or diagnostic procedures
Collaborates with the Licensed Medical Physician in formulating plans of care for assigned patients.
Works with the patient care team to carry out the agreed upon plan of care
Administers medication in accordance with policy and procedure utilizing knowledge base to assure patient safety
Accountable for communicating all changes in the patient’s biophysical, emotional, and spiritual status to the Licensed Medical Physician
Promotes and has knowledge of and utilizes appropriate patient care protocol relating to the physical and psychological needs of neonatal, pediatric, and adolescent patients
Consistently communicates with registered nurse, multidisciplinary team members and physician to enhance quality of patient care and unit activities

Required Qualifications:

Currently licensed as a registered professional nurse in the State of Illinois
A Bachelor's degree in Nursing
- Associate Degree Nurses are required to enroll in a BSN program within six (6) to nine (9) months of hire, and make progress toward and complete their BSN degree within 30 months. ADN nurses receive 100% tuition reimbursement towards a BSN degree from an accredited program.
Two to three years’ experience as a registered nurse
Pediatric experience as RN preferred
Active BLS certification from the American Heart Association (AHA)
