Responsibilities

As a Crisis Response RN, you’ll support high-acuity areas, to facilitate timely interventions, patient stabilization, and staff development. 

  • Respond to patients in clinical crisis and lead stabilization efforts until appropriate care placement is achieved 
  • Collaborate with care teams to assess complex cases while guiding and coordinating clinical decision-making 
  • Assist with monitored patient transports and escalation of care across critical units 
  • Mentor and educate staff on evolving processes, protocols, and high-acuity patient management — serving as a resource for clinical best practice

Qualifications

Education & Experience 

  • High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required 
  • 2 years of nursing experience of which at least 1 year in an Intensive Care Unit (ICU) or Emergency Department required 

Licenses and Certifications  

  • RN - Current and valid Registered Nurse license to work within the state of California required

The Clinical Case Consultant works closely with Lead Care Managers, Community Health Workers, Licensed Vocational Nurses, Behavioral Health staff, healthcare providers, health plans, hospitals, and community-based organizations to ensure members receive coordinated, high-quality, person-centered care. This role combines clinical expertise, care coordination, quality improvement, staff development, field-based oversight, and community collaboration to improve health outcomes, reduce avoidable utilization, and support compliance with CalAIM ECM requirements and best practices. Communicates with community health related persons to coordinate the care plan. POSITION QUALIFICATIONS 1. Graduate of a National League of Nursing accredited school of nursing. 2. Current licensure in State of CA and CPR certification. 3. Registered nurses / Licensed Vocational nurse shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN / LVN license. Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 6. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. 7. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
