Hours would be: 6:45pm-10pm or 7:00am-11am
RESPONSIBILITIES:
Resident Care (PHI)
Provide a balance of emotional and physical support that allows an individual to function at their highest level of independence. This will include variations of the following:
Dressing
Bathing
Grooming
Eating/Feeding
Bed Mobility/Positioning
Ambulation/Wheelchair mobility
Range of Motion
Skin Care
Continence Care/Toileting
Behavioral Support
Providing Appropriate Rest Periods
· Maintain a safe resident environment by assuring:
Resident room is free and clear of clutter
Telephones are within reach and cords out of walking range
Brakes are in the locked position on beds
Beds are in the low position
Personal Alarms and Siderails are used only as ordered
Appropriate use electronic lifts appropriately
Assures that one side of halls is free of obstructions.
Safety rounds on assigned team are performed at least every 2 hours
· Communicates changes in residents’ condition and function to nurse immediately when noted. Reports reduced appetite, reduced fluid intake and lack of a bowel movement for 3 days to nurse immediately. Also, report any skin breakdown, redness or rashes and bruises.
· Performs Assigned Tasks as directed by the Unit Nurse
Take Vital Signs
Passes Fresh Ice Water at least once per shift and as directed
Passes Juice and Snacks between meals per policy
Weigh residents no later than 5th day of every month and as needed
Assist in admission, discharge, and transfer of residents
· Accountable for completion of C.N.A. documentation which includes progress
notes, percentage of meals and fluid taken, bowel sheet, and ADL’s.
· Reposition residents every 2 hours who are high risks for pressure ulcers.
· Maintains Clean Environment by assuring:
Meal trays have been removed from dining areas, and resident rooms. Makes beds for bed bound residents and isolation residents. Maintains a clean shower and bathing area.
Takes laundry bags to soiled utility room when full.
Maintains a tidy resident closet, checks and labels clothing with residents’ name.
· Communicates with residents, families, significant others, and co-workers utilizing
a pleasant positive approach.
Regulatory Compliance (PHI)
· Respects Residents Rights for Privacy and Dignity.
· Reports all suspected, alleged, and/or witnessed verbal, physical, or sexual abuse immediately to nurse.
Leadership
· Service a preceptor and positive role model for new employees.
Continuous Improvement
· Ensures standard precautions and infection control, isolation, fire, safety, and sanitation practices and procedures are followed by co-workers.
· Participates in Briarwood specific and organization wide Continuous Improvement (CI) initiatives. Participates on CI teams as requested.
Training and Development
· Attends all staff meetings and in-services designated as required or mandatory.
· Attends organization wide program and meetings to be informed and current on organization strategies and programs.
· Maintains competence with specific C.N.A. assistant skills by performing competency training as directed by the DON.
ENVIRONMENATL AND PHYSICAL DEMANDS:
Physical Activities:
Constantly standing, walking, talking, hearing, squatting, bending, squatting, reaching, twisting, driving, feeling, and grasping.
Occasionally sitting, crouching, crawling, reaching hands and arms, tasting, and smelling.
Range of Motion:
Constantly moving from waist-shoulder height and waist and below.
Frequently moving from shoulder and above height.
Weight Lifting Requirement:
Constantly being able to lift up to 75 lbs.
Vision Requirements:
Frequently utilizing peripheral and depth perception.
Occasionally looking at computer monitor and utilizing color vision (identifying and distinguishing colors).
Environmental Exposure:
Constantly being exposed to body fluids/blood and frequently operating equipment and wearing protective clothing.
QUALIFICATIONS/REQUIREMENTS:
Education/Licensure:
· Current State of Illinois C.N.A. Certificate
· Current CPR card
Experience:
· May be new graduate or pending certificate when staffing patterns can accommodate.
Knowledge, Skills, Abilities:
· Ability to read and write.
· Ability to communicate with residents and associates.
· Ability to remain calm under stressful situations.
· Exposure to communicable disease.
· Must be able to work with ill, disabled elderly, emotionally upset, and at times, hostile people within the facility. (PHI)
· Ability and willingness to work more than usual hours when needed, possibly during different shift, weekends, or holidays.
· Capable of maintaining regular attendance.
· Meet all local health regulations and pass post offer pre-employment physical exam, if required, which may include drug screening, criminal background investigation and reference inquiry.
· Capable of performing the responsibilities and essential job functions with or without reasonable accommodation.
· Computer literate.
