Summary Job Summary: The Peritoneal Dialysis Program Coordinator RN is responsible for supporting the development, implementation, and ongoing operations of the peritoneal dialysis clinic. This role provides clinical coordination, patient and caregiver education, regulatory readiness support, and interdisciplinary collaboration to ensure safe, high-quality home dialysis services. The Peritoneal Dialysis Program Coordinator RN must demonstrate strong knowledge of peritoneal dialysis standards of care, CMS Conditions for Coverage for ESRD facilities, home dialysis training and monitoring requirements, infection prevention practices, documentation expectations, and quality assessment and performance improvement activities. The Peritoneal Dialysis Program Coordinator RN serves as a clinical resource for patients, families, providers, and staff while exemplifying the Rush mission, vision, and values and acting in accordance with Rush policies and procedures. Other Information Required Job Qualifications: Current registered nurse licensure in the state of practice. Bachelor of Science in Nursing (BSN) required; the BSN requirement will be waived for candidates with an Associate Degree in Nursing and a minimum of 25 years of experience in nephrology and peritoneal dialysis. Current CPR/BLS certification. Demonstrated knowledge of peritoneal dialysis clinical practice, patient assessment, plan of care coordination, infection prevention, home dialysis training, and patient/caregiver education. Working knowledge of CMS Conditions for Coverage for ESRD facilities and regulatory expectations related to home dialysis services, documentation, patient training, monitoring, and quality improvement. Analytical ability to identify trends, evaluate patient safety concerns, review documentation, and support quality improvement activities. Strong communication, teaching, coaching, and collaboration skills to support patients, caregivers, clinic staff, and interdisciplinary partners. Ability to interact effectively with patients, families, and team members in complex or difficult situations. Ability to review records for accuracy, use electronic health record systems, enter and retrieve data, and closely examine clinical documentation, results, reports, and patient-monitoring information. Preferred Job Qualifications Previous experience supporting the opening, expansion, or regulatory preparation of a dialysis, home therapies, or ambulatory specialty clinic. Certified Dialysis Nurse, Certified Nephrology Nurse, or related nephrology/dialysis certification. Coordinates with the interdisciplinary team to ensure comprehensive patient assessment and individualized plan of care development. Coordinates and/or provides peritoneal dialysis education and training for patients and designated caregivers prior to initiation of home dialysis and when modality, caregiver, or patient needs change. Ensures patient and caregiver training addresses kidney failure management, peritoneal dialysis technique, equipment and supply use, medication administration as applicable, recognition and response to complications, self-monitoring, emergency procedures, infection control precautions, and waste storage/disposal expectations. Documents patient and caregiver training, return demonstration, competency, and comprehension in accordance with regulatory requirements, organizational standards, and clinic procedures. Retrieves, reviews, escalates, and documents home dialysis self-monitoring data and other relevant patient information to support timely intervention, patient safety, and continuity of care. Establishes caring, therapeutic relationships with patients and families through effective communication, acknowledgement of patient/family perceptions of health and illness, and mutual goal setting for optimal patient experience and outcomes. Coordinates care using the nursing process, multidisciplinary collaboration, and appropriate health services and community resources across the care continuum. Recognizes patterns, complications, and changes in peritoneal dialysis patients that require evaluation, escalation, or modification of the plan of care. Utilizes evidence-based ambulatory and dialysis nursing interventions during clinic visits, telephone encounters, electronic communications, and home dialysis support activities according to state regulations, regulatory agency standards, and organizational policies and procedures. Applies infection prevention and control principles specific to peritoneal dialysis, including aseptic technique, catheter/exit-site care education, peritonitis prevention, environmental considerations, and patient safety practices. Participates in Quality Assurance and Performance Improvement (QAPI) activities. Partners with physicians, advanced practice providers, nurses, dietitians, social workers, pharmacists, operations leaders, supply vendors, and other stakeholders to support coordinated, patient-centered peritoneal dialysis care.
