
MDS Coordinator - RN/LPN BR PACE Center
Job Description
The MDS Coordinator will conduct a comprehensive assessment (Minimum Data Set Questionnaire) of all Participants prior to enrollment and on an annual basis according to State Guidelines, will input information into the MDS into the State OPTS system and develop or update the Participant Plan of Care in the electronic medical record.

QUALIFICATIONS
Experience - 3 years clinical experience as an LPN / RN
Education - Diploma or Associate's Degree from accredited LPN Program or current Louisiana State license as RN
Licensure - BLS, Licensed Practical Nurse or Registered Nurse

- Initial Enrollment:
- Perform a comprehensive assessment of potential Participants by the 15th of month to include a pre-med questionnaire, a physical exam, observation, and input from family/caregiver.
- Complete the MDS and submit through the State OPTS system.
- Develop a Pla of Care and attend the Pre-enrollment Meeting.
- Attend the Enrollment meeting to review Plan of Care and complete regulatory documents with potential enrollee and family/caregiver.
- Schedule and coordinate new enrollee meeting with Interdisciplinary Team for assessment completion after enrollment.
- Attend the IDT meeting after enrollment for final comprehensive, interdisciplinary Plan of Care.
- Annual Assessment:
- Perform a comprehensive assessment prior to the end of the month to include physical exam, observation, input from the Interdisciplinary Team, and input from family/caregivers.
- Complete the MDS and submit through the State OPTS system.
- Track and complete Participant eligibility for Permanent Waiver and complete paperwork to the State of Louisiana.
- Report to assigned RN Case Manager MDS findings.
- Documentation of assessments and any communications with participant/caregiver/family in the Electronic Medical Record. Report to Center Manager / Senior Director any omissions/trends with MDS/Care Plan process involving other departments.
- Completion of ancillary forms as needed (Tuberculosis Assessment Screening, diabetic foot screening).
- Develop and revise monthly Provider schedule outlining initial, 6 month, and annual assessments. Distribute to Interdisciplinary Team.
- Develop and schedule monthly IDT (Plan of Care) meetings.
- Maintains appropriate records and provides feedback, validation, and quality reports. Conducts edit checks weekly, making any necessary changes, while providing follow-up to administration and staff.
