The Nurse Practitioner will perform all duties and tasks under the medical direction of an anesthesiologist. The Department of Anesthesiology & Perioperative Care's division of Center for Perioperative Care advocates for each patient to be medically optimized prior to surgery, to experience a smooth hospital course, and to return back to normal life activities. Patients receive individualized service that is expertly coordinated across multiple departments and phases of care.

Your Role on the Team

Work with the physicians/surgeons/APP colleagues and other clinical and administrative staff to ensure efficient patient management based on accepted practice, including but not limited to, performing histories and physicals, pre-operative planning/coordinating with anesthesia/outside providers and PCPs as applicable, responding to change in patient status, providing needed prescriptions/evaluations/referral/treatments as permitted under their standardized procedure. Provide home health services and DME, DMV evaluations, referrals, consultations, PT, and order radiographic or diagnostic requests, specialized treatment privileges (e.g. chemotherapy with the approved privilege). May also provide patient education, arrange follow-up appointments, and engage in any approved privileges the APP has completed the necessary proctoring or FPPE through prior to competency for independent practice and procedural activity.

What You'll Do Conduct 5-6 Annual Wellness Home Visits per day within the San Bernardino County area Conduct 10-11 Annual Wellness Telehealth Visits per day across Astrana Health’s covered markets Document chief complaint, previous medical, family, and social history, review of systems, examination, assessment, and appropriate plan of care Identify gaps in care during the evaluation that may require additional screening, diagnostics, and laboratory studies Recapture previous conditions while carefully capturing newly found conditions Create or update a Personalized Prevention Plan based on current health and risk factors to optimize the management of current conditions and prevent illnesses Provide patient education regarding preventive services and prescribed treatments, reinforce the importance of staying current on covered preventive services Make recommendations for member's follow-up with their primary care physician Communicate with AWV Care Coordination team to ensure patients receive necessary referrals to additional services Complete documentation/charting in an accurate and timely manner Comply with federal, state, and local legal and professional requirements by studying existing and new legislation, anticipating future legislation; enforcing adherence to requirements; advising management on outstanding actions Other duties as assigned Qualifications Active Nurse Practitioner Licensure, Active Nurse Practitioner Furnishing License OR Active Physician Assistant Licensure Current Board Certification with (AANP, ANCC, NCCPA) Valid DEA and NPI You’ll be great for this role if you have: Minimum one year of direct patient care experience as a Nurse Practitioner OR Physician Assistant in Primary Care, Gerontology, or related field Experience in HEDIS and HCC as it pertains to AWV/Medicare/ACO population Experience with EHRs Fluency in Spanish (preferred) Environmental Job Requirements and Working Conditions This is a full-time, hybrid position scheduled as four 10 hour shifts per week or five 8 hours shifts per week, serving patients throughout the San Bernardino County territory. The national target compensation target pay range for this role is between $140,000 - $150,000 annually. Actual compensation will be determined based on geogrpahic location (current or future), experience, and other job-related factors.
