A Brief Overview

Becoming an RN, in Long Term Care (LTC), allows you to provide significant, ongoing care to elderly patients. You can develop deep connections with residents and their families, getting to know their stories and personalities over time, while making a positive impact on their quality of life. By providing attentive care, you can significantly improve the daily lives of residents, making a noticeable difference.

What you will do:

  • Regularly evaluating residents' physical and mental health, including vital signs, cognitive function, and overall well-being.
  • Accurately dispensing and monitoring medications according to physician orders.
  • Maintaining open communication with residents, families, physicians, and other healthcare team members to ensure coordinated care.
  • Observing for any significant changes in resident health status and promptly reporting them to the appropriate healthcare provider.
  • Assessing pain levels and administering pain medication as prescribed.
  • Providing emotional support and addressing psychosocial needs of residents.
  • Maintaining accurate and detailed resident records, including progress notes and medication administration logs.
  • Recognizing subtle changes in resident behavior or health status due to the chronic nature of conditions.
  • Collaborating effectively with Certified Nursing Assistants (CNAs) and other healthcare professionals to deliver comprehensive care.
  • Comply with all aspects of resident rights including confidentiality and privacy.
  • Lead, by example, following Freeman Regional Health Services CARE standards.

Required Education, License/Certification, or Work Experience:

  • Registered Nurse (RN) - SD Board of Nursing
  • 1-3 years experience working in a clinical or long term care environment, preferred.

Performs patient assessments, hourly rounding and initial nursing assessment of all new admissions, admission note as well as on-going assessments and patient notes. Coordinates making physicians rounds and physician notifications with the Resource Nurse. Carries out new orders and/or communicates new orders to team members to be completed. Makes necessary observation and suggestions to attending physicians. Accountable for acknowledging all new orders and following through with them. Provides bedside report for oncoming staff. Provides bedside care and functions to the patients. Is responsible for appropriate discharge and patient teaching for their shift and completes discharge routine documentation. Knows the hospital area resource services and insurance certification procedure for patients. Administers medication and treatments per physician orders and reviews H & P, meds, treatments, etc. Utilizes principles of good leadership and management in organizing nursing care on the unit. Serves as liaison between patient, physician, and staff. Attends and conducts in-services meeting, if needed.
