Job Responsibilities (listed in order of importance and/or time spent)

  • Competent in the clinical management of home infusion patients including Patient management skills, the Nursing Process, the rules of Federal, state, and local regulatory entities that oversee and control the provision of care in a Home Health Agency.
  • Knowledgeable of documentation and billing requirements for home health/ infusion care goods and services.
  • Determines suitability of patients for home care.
  • Supervises, monitors, and evaluates the quality and effectiveness of patient care, including, when appropriate, in home evaluations of field staff.
  • Reviews and monitors appropriateness of Nursing Care Plans and directs the staff to modify the plan as needed.
  • Acts as Administrator and/or Director of Nursing of the Home Health Agency for Medicare certified home health agencies in the absence of a director or Manager of Nursing.
  • Manages daily activities with regards to effective operations and communications and ensures the office remains in compliance with all regulating and accreditation entities. Complies with reporting requirements, ensures policy and procedure, compliance and monitors and evaluates medical supply usage.
  • Participates in the process for assessing, managing, and reporting nursing operational and financial data to the Regional Nurse Manager, Area Director of Nursing, and/or Director of Operations. Supports the development and implementation of the Plan of Care for each patient.
  • Develops, implements, evaluates, and participates in office specific continuing education and orientation programs. Assists the Director of Operations, Area Director of Nursing and/or the Nurse Manager in the recruitment, development, and monitoring of new staff. Coordinates and participates in on-call responsibilities. Maintains confidentiality of patient and proprietary information.
  • Participates as a member of or directs the professional advisory committee, infection prevention and process improvement teams, including conducting chart audits. Identifies opportunities for improvement and communicates to the Nurse Manager, Area Director of Nursing and/or Director of Operations. Speaks knowledgeably of the scope of services that are available through Option Care and provides support to sales professionals on an ongoing basis.

Basic Education and/or Experience Requirements

  • Registered Nurse with a license in the current state of practice with 1 year of nursing supervisory/charge experience, or at least 3 years of relevant nursing experience with demonstrated progression.
  • Required certification in CPR.
  • Member of a state or national professional organization.

Basic Qualifications & Interests (BQIs)

  • Access to a reliable means of transportation, which will enable the incumbents to make home visits to treat/attend to their patients. If such means of transportation would include a personal vehicle, a valid driver’s license and a proof of insurance would be required.
  • Experience applying knowledge of local, State, Federal, accrediting body, and OSHA rules and regulations.
  • Experience providing customer service to internal and external customers, including meeting quality standards for services, and evaluation of customer satisfaction.
  • Experience in the indirect management of team members, including assisting in the development, training, and assignment of work/projects to other members of a team.
  • Experience using time management skills such as prioritizing/organizing and tracking details and meeting deadlines of multiple projects with varying completion dates.
  • Basic level skill in Microsoft Word (for example: opening a document, cutting, pasting, and aligning text, selecting font type and size, changing margins and column width, sorting, inserting bullets, pictures and dates, using find and replace, undo, spell check, track changes, review pane and/or print functions).
  • Basic level skill in Microsoft Excel (for example: opening a workbook, inserting a row, selecting font style and size, formatting cells as currency, using copy, paste and save functions, aligning text, selecting cells, renaming a worksheet, inserting a column, selecting a chart style, inserting a worksheet, setting margins, selecting page orientation, using spell check and/or printing worksheets).
  • Basic skill level in Microsoft PowerPoint (for example: inserting, rearranging, hiding, and deleting slides, navigating between slides, increasing list level, adding, centering and editing text, changing views, inserting a table or a note, moving objects, printing outline view and/or running a slide show).
  • Experience developing ways of accomplishing goals with little or no supervision, depending on oneself to complete objectives and determining when escalation of issues is necessary.

Travel Requirements: (if required)

Willing to travel at least 50% of the time for business purposes (within state and out of state).

Preferred Qualifications & Interests (PQIs)

  • BSN
  • CRNI or other professional certification.
  • At least 1 year of supervisory experience in a hospital or home health care setting is preferred.
  • At least 1 year of experience in the indirect management of team members, including assisting in the development, training, and assignment of work/projects to other team members is preferred.

Operations Key Responsibilities Plan, organize, and delegate activities as required for safe, efficient, and effective operation of the unit including active participation with other hospital departments and leadership to accomplish this purpose. Provide resources and guidance to support the utilization of evidence based practice standards. Identify and utilize processes and resources that support optimal delivery of data and information to staff. Facilitate activities that prepare unit, environment, and staff to meet credentialing bodies requirements. Identify key drivers in throughput process, evaluate areas role, and develop solutions to barriers. Quality Patient Care Outcomes Plan, manage, patient care processes to improve performance. Facilitate delivery of quality patient care. Require documentation that meets hospital and credentialing standards/policy/guidelines. Demonstrates positive supportive role in change process. Human Resources Responsible for staffing based on patient care needs. Responsible for personnel functions including staff, performance reviews and corrective action. Promote professional growth and development of all employees. Recognize and reward individual and team accomplishments. Promote staff retention. Facilitate new staff orientation. Supports shared decision-making through the unit-based council. Customer Satisfaction Incorporate customer needs and expectations into daily operations and business planning strategies. Promote team building strategies and principles of group process in interaction with staff. Budget Prepares operating and capital budgets for areas of responsibility. Manages approved operating and capital budgets effectively and adjusts strategies as needed to meet budget expectations. Identifies and evaluates patient care requirements and outcomes based upon patient acuity, system and budgetary guidelines. Professional Accountability Demonstrates involvement in professional organization. Participates in hospital/department/system committees. Stays current with professional activities. Alerts appropriate individuals regarding ethical and legal concerns (i.e. Risk Manager, Compliance Officer, Director). Educates, promotes and evaluates appropriate use of chain of command by staff. Acts as a role model/mentor. Adheres to ANA Code of Ethics. Complies with hospital policies and with Nurse Practice Act. Active in Reach Out THR and other community activities. Makes appropriate referrals to Peer Review. Participates in 360 degree evaluations. Creates development plan that identifies and addresses areas of needed growth. Completes reports in a timely manner. Utilizes various resources for development. Education Bachelor's Degree Nursing Required Other - CLIAFE Must comply with CLIA regulations regarding foreign education equivalency Required Experience 3 Years Experience as a registered nurse with previous experience as a charge nurse or similar leadership role (i.e., charge nurse, committee chair, preceptor, supervisor) Required Emergency Experience Strongly Preferred 5 or more years of Nursing Experience Strongly Preferred Supervisory Or Managerial Experience Strongly Preferred Licenses and Certifications RN - Registered Nurse Upon Hire Required And BCLS - Basic Cardiac Life Support prior to providing independent patient care and maintained quarterly Required And PALS - Pediatric Advanced Life Support if working in ED within 90 Days Required And ACLS - Advanced Cardiac Life Support if working in ED within 90 Days Required And TNCC - Trauma Nursing Core Course if working in ED within 90 Days Required Skills Knowledge of industry standards:organization, theory, principles, and research processes, quality improvement and change management techniques, financial analysis, budget process and planning, accreditation and regulatory standards, strategic planning principles, ability to assess nursing care requirements based on nursing principles, scope of practice, and regulatory standards.
