  • UT Health Carthage located in Panola County, boasts 23 private patient rooms. The hospital also features a 24-hour emergency department, designated as a Level IV trauma center and acute stroke ready facility with advanced diagnostic technology, a cardiac rehabilitation program, GI lab, outpatient clinics and a sleep lab.

Responsibilities

  • The Case Manager RN is responsible for assessing and coordinating patient care across the continuum, utilizing accepted criteria and guidelines to manage benefits and determine the medical necessity of the services provided.
  • This position functions to achieve optimal clinical and quality outcomes by effectively managing care and resources to reduce unnecessary utilization.
  • The RN Case Manager reports to the Director of Case Management or designee. 

Qualifications

Job Requirements:

  • RN, Associates degree in nursing or higher

 Preferred Job Requirements:

  • 2+ years’ acute hospital-based nursing experience
  • Experience in acute care hospital case management
  • Certification in case management (ACM or CCM)

Complete incident report if any occur during shift Complete lab inquiries and noted on lab log for any new orders or any labs received (log them in).

This role serves as the primary escalation point for virtual nurses, virtual PSAs (Patient Safety Attendants), and Surveillance Monitoring Staff. Using multiple virtual care and monitoring platforms, the Virtual Care Charge Nurse oversees staffing, workflow prioritization, and patient safety escalations to support safe, efficient, and compliant virtual care delivery across assigned departments. Maximizes the effectiveness of virtual nurses, virtual PSAs, and surveillance monitoring staff during the assigned shift. Serve as a resource for patient safety concerns, workflow issues, and technology-related barriers from virtual and bedside teams.
