Role Overview As a Home Health RN, you are the clinical lead for your patients - managing the plan of care, coordinating across disciplines, and driving outcomes from admission through discharge. You’ll combine advanced assessment skills, care planning, and patient education to reduce hospitalizations, improve recovery, and support long-term independence at home. Enliven is seeking an experienced RN (Registered Nurse) to service home infusion and Infusion Center patients in Chandler and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patients' well-being by providing high-quality nursing care. Responsibilities: RN (Registered Nurse) Complete daily electronic forms for each assigned patient within a secure Electronic Medical Record (EMR) System. Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients. Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager. Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment.
