Aspire Senior Living Pleasant Hill is seeking a detail-oriented Long-Term Care MDS Coordinator to manage and ensure the accuracy and compliance of the Minimum Data Set (MDS) assessments for residents.
MDS Coordinator:
As an MDS Coordinator, you will play a vital role in ensuring the accuracy and compliance of MDS assessments and documentation, collaborating with interdisciplinary teams for care planning, and monitoring resident care plans for accuracy and effectiveness. MDS Coordinator will be responsible for maintaining compliance with state and federal regulations, which requires strong organizational and communication skills.
MDS Coordinator Key Responsibilities:
• Complete and oversee MDS assessments and documentation
• Ensure timely submissions to CMS
• Collaborate with interdisciplinary teams for care planning
• Monitor resident care plans for accuracy and effectiveness
• Maintain compliance with state and federal regulations
MDS Coordinator Qualifications:
• Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure in Missouri
• Must have experience in long-term care and MDS coordination
• Strong organizational and communication skills
• Proficiency in MDS software and CMS regulations
