Position Summary
The Licensed Practical Nurse (LPN) (Float) provides direct patient care in accordance with applicable laws and organizational policies. The LPN supports the delivery of compassionate, high-quality care in hospice and palliative care settings, performing assigned clinical tasks and collaborating with interdisciplinary teams to ensure optimal outcomes for patients and caregivers.
Essential Functions
- Assist in implementing and modifying patient care plans in collaboration with the RN and interdisciplinary team.
- Deliver patient-centered care, including administering medications and performing procedures within the LPN scope of practice.
- Monitor and report changes in patient condition to the RN and physician; document accordingly.
- Educate patients and caregivers on appropriate self-care practices.
- Participate in discharge planning and team meetings.
- Maintain patient confidentiality and ensure compliance with HIPAA regulations.
- Manage and order medical supplies as needed.
- Facilitate communication between care team members and physicians.
- Support organizational initiatives such as disaster preparedness and performance improvement activities.
- Administer IV therapy as permitted by licensure and training.
- Follow general safety and infection control procedures.
- Participate in total organization program activities including:
• Demonstrating professional relationships with internal and external customers
• Promoting the organization’s image
• Using chain of command for problem solving
• Demonstrating appropriate dress and grooming
• Displaying courtesy, tact, patience, and respect
• Maintaining good attendance and notifying of absences per policy
• Supporting all departments cooperatively
• Participating in QAPI activities
• Following Clinical Care standards
Knowledge and Critical Skills
- Knowledge of hospice nursing practices and principles
- Strong patient observation, documentation, and communication skills
- Ability to adapt to changing environments and schedules
- Excellent interpersonal and organizational abilities
- Capable of working with diverse populations across age, race, and health conditions
- Demonstrates sound judgment and maintains confidentiality
Experience
- Minimum of two (2) years of patient care experience required
- Acute care and hospice nursing experience preferred
License / Certification
- Current Florida LPN license
- IV Certification (Florida preferred)
- Current CPR certification
- Graduate from an accredited Florida Board of Nursing program for Licensed Practical Nurses
- Must be willing to work at various hospice centers or teams as needed
- Participation in organizational quality and safety programs is expected
