Job Summary

Our Primary Care practice is located at 65 Calef Highway, Suite 200 Lee, NH 0386. We offer personal and compassionate care to help individuals and families achieve their best health.

We are seeking a full-time, 40-hour Triage Registered Nurse to support our Family Practice team. This role is telephone-based triage, supporting patients across the lifespan, from pediatrics through adults. The ideal candidate will have prior telephone triage experience, with experience in primary care and pediatrics strongly preferred.

During the first 30–90 days of training, the schedule will be five 8-hour shifts per week, Monday through Friday 8:30 AM to 5:00 PM. Following successful completion of training, the schedule will transition to four 10-hour shifts, Tuesday through Friday, from 7:00 AM to 5:30 PM.

This position will transition into to a fully remote model after the initial 90-day onsite training period.

Summary
Responsible for patient care and support within the outpatient setting through providing compassionate care, assisting in procedures, and collaborating effectively in a dynamic ambulatory care environment.

Essential Functions
-Deliver direct patient care and support, ensuring high nursing standards.
-Conduct patient assessments, devise care plans, and assist in outpatient procedures with a focus on patient comfort and safety.
-Educate patients and families on treatment plans, medications, and post-procedure care.
-Collaborate closely with healthcare professionals to coordinate comprehensive patient care.
-Administer medications, perform IV therapy, and maintain accurate patient records in compliance with regulations.
-Uphold infection control protocols and maintain a safe environment for patients and staff.
