The Licensed Practical Nurse will be responsible for, but not limited to:

Day-to-day coordination and oversight of the nursing process for residents assigned to his or her care.
Direct and indirect provision of care according to each resident's nursing and medical plan of care and in accordance with state and federal regulations and accepted nursing standards of practice.
Supervises administratively in the building assigned, Certified Nursing Assistants and licensed nurses of all staff of his/her respective discipline.
Supervision follows all accepted state and federal guidelines for the respective discipline.

Position Requirements

Education Graduate from credentialed school of nursing
Certification/Licensure: Currently licensed as a nurse in the state of practice.

Performs any and all tasks necessary to improve the quality of care, to improve the productivity of medical staff, and to improve participants satisfaction with the medical/nursing care they receive. Performs any and all tasks necessary to enhance reimbursement and reduce denials for services rendered. Performs phlebotomy, EKGs, vital signs and assists in urine collection. Position Summary Support the referral, screening, and onboarding process for potential and newly admitted hospice patients. This role assists in reviewing incoming referrals, gathering essential clinical information, and coordinating with physicians, referral sources, patients, and families to facilitate timely hospice admissions.
