This position is a 100% onsite opportunity. You will need to be based in the greater Chicagoland area. Located in Chicago’s Streeterville neighborhood, UChicago Medicine River East is a state-of-the-art outpatient center featuring a wide range of care services. In addition to those listed below, we offer infusion therapy, lab services, and radiology services, including CT, MRI, mammography, X-ray, DEXA scan and ultrasound. A Registered Nurse, UCM Care Network, is responsible for providing professional nursing care for patients including assessment, treatment, care planning, and medical care evaluation by following established standards, protocols and practices. Assists physicians and staff in coordinating patient care. Functions as primary liaison between patients, staff and physicians. This position will be located in a community-based physician practice. Essential Job Functions Obtains patient consent for care and ensures patient confidentiality Obtains and documents health history, information on chief complaint, vital signs, and health risk factors and assesses disease condition Provides patient care based on practice guidelines, standards of care, and federal/state laws and regulations Executes prescribed treatments and medical interventions, administers prescribed medications, and monitors and documents treatment progress and patient response Delegates as allowed under the Nurse Practice Act. Under the general direction of the Heart and Vascular Administration , the Clinical Practice Manager is responsible for administrative and patient care activities of the ambulatory physician practice at the Duchossois Center for Advanced Medicine (DCAM). The nurse manager oversees day-to-day operations, including scheduling, planning, organizing, and directing all activities related to patient-centered care in the practice, and handles an array of administrative duties, including recruitment and hiring; supervision; purchasing; inventory; facility, safety and equipment management. Responsible for preparing for and participating in various inspections, including the JCAHO accreditation process. The incumbent will focus on the patient and family experience, quality and safety, coordination of care across the continuum, use of technological tools and solutions including EPIC, metrics reporting, customer service, and oversees the budget, billing and related revenue cycle services. In this role you will partner with the rest of the clinical team to provide patients the highest quality of care. In this role, the Nurse Associate (RN) is responsible for coordinating diagnostic and therapeutic regimens for a specified group of patients, and maintains cohesiveness between clinical research, physicians, and patients. The Nurse Associate coordinates physician activities with referring physicians, nursing staff, hospital administration, ancillary services, support services, insurance companies and other departments/sections and agencies as needed, and assists the physician with a wide range of activities necessary to manage clinical research programs.
