The Behavioral Health Clinical Nurse Department Pool I is responsible for: Independently caring for a group of patients consistently using the nursing process and clinical standards. Delegating appropriate patient care activities to other members of the health care team. Professional accountability and development. Inform team assistant of any schedule changes and communicates caseload and patient care issues to Supervisor Patient Care. Coordinates care with care team assigned to caseload, participates in care coordination communication and documentation thereof. Self manages visits weekly by triaging by risks associated with potential rehospitalizations. Participates in quarterly performance improvement action plans. Complete patient documentation timely as indicated by supervisor with accuracy. Minimum Qualifications: Other information: Required 1 year - Registered Nurse; Or - 1 year - Licensed Practical Nurse; Preferred 1 year - Home Care.
