Summary

The Registered Nurse – Patient Navigator – MHMG MMCP role guides patients through the care continuum, establishing contact with recurrent and newly diagnosed cancer patients and/or family members, or caregivers. The Patient Navigator will understand the clinical pathways established by the Oncology Program and will direct the patients to health care services within the organization, at outside facilities, and within the community for timely diagnosis, treatment and survivorship.

Required Minimum Knowledge, Skills, And Abilities (KSAs)
  • Education: Graduate from an accredited School of Nursing required. Bachelor’s Degree in Nursing required or must complete BSN within three years of start date.
  • License/Certifications: Current applicable state(s) license as a Registered Professional Nurse required. Oncology certification or defined process for achieving certification required.
  • Experience: Experience working with oncology patients and families preferred. Experience in community outreach preferred.

This specialized team provides compassionate, expert care for patients with primary brain tumors and central nervous system (CNS) malignancies, including glioblastomas, astrocytomas, oligodendrogliomas, CNS lymphomas, meningiomas, metastatic brain disease, spinal tumors, neurofibromatosis-related tumors, and other complex neuro-oncology conditions. The Triage RN plays a vital role in coordinating and managing patient care throughout the treatment journey. In this position, you will collaborate closely with a multidisciplinary team that includes Medical Assistants (MAs), Clinical Team Assistants (CTAs), Advanced Practice Providers (APPs), attending physicians, IV Therapy staff, authorization specialists, NorDx laboratory personnel, and other healthcare professionals. Together, we work to provide timely triage, seamless care coordination, and exceptional patient-centered oncology care. While experience in both oncology and triage nursing is strongly preferred, we welcome candidates with triage experience from other specialties who are eager to learn and grow within neuro-oncology. We are seeking an experienced Registered Nurse to provide ambulatory nursing support, including telephone triage, management of answering service messages, patient outreach, medication refill processing, and care coordination for our Scarborough Neurology practice.
