The Resource Nurse is responsible for the coordination of clinical activities through developing and sustaining an environment that promotes excellence in clinical practice and customer service, while striving for optimal efficiency, productivity and utilization of all resources. Utilizes the nursing process, leadership skills and current evidence based clinical knowledge to assess the health needs of guests, collaborate with health team members to coordinate guest care, anticipates outcomes of nursing interventions, and sets criteria for the quality of patient care. Resource Nurse may also assume other duties as a licensed nurse, as necessary.

Responsibilities

  • Works within the interdisciplinary team structure to complete the guest’s initial Plan of Care at the time of admission.
  • Assist with the planning, implementation and evaluation of the guest care area/unit orientation.
  • Utilizes material, equipment and time in a safe, beneficial, and cost-effective manner.
  • May perform other duties as assigned.

Qualifications

  • A graduate of an accredited school of nursing.
  • Prior to date of hire, must hold a current, valid nursing license (Licensed Practical Nurse or Registered Nurse) through the Arizona State Board of Nursing, or hold a compact RN or LPN license. Must maintain valid nursing licensure throughout the course of employment.
  • CPR certification required at time of hire.
  • Fingerprint Clearance Card required.
  • Must have a minimum of three (3) years LPN experience in an LTC/SNF/Rehab setting.
  • If an LPN, the ability to obtain RN licensure through the AZ State Board of Nursing within six (6) months to one (1) year of hire, is highly preferred.OR
  • Must hold current RN licensure through the AZ State Board of Nursing AND have a minimum of one (1) year experience in an LTC/SNF/Rehab setting.

POSITION SUMMARY This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. MINIMUM QUALIFICATIONS Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice. Relates throughout the interview process the experience, training and education needed to perform the job. Experience in the clinical area for which he or she is applying is desired.
