On an Average Day You Will:
(includes, but not limited to)

Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children
Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record
Manage health conditions and terminal illness by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating hospice care services
Complete the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations
Provide clinical information to the hospice medical director for interpretation of eligibility for hospice recertification
Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners
Assure that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members
Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician
Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols
Collaborate with the interdisciplinary group to create, review and revise the patient's plan of care
Fulfill the obligation of requested and/or accepted assignments
Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues
Complete all other duties as assigned

Requirements:

Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program
Must have current nursing licensure in State
Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience
Must have accreditation from an approved certifying body for advanced practice nursing as required by the State
Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
Must be a licensed driver with licensed automobile that is insured in accordance with
organization/state requirements and is in good working order

The Cardiothoracic Surgery Advanced Health Practitioner position has clinical, procedural, administrative, and educational responsibilities in the care of adult patients requiring cardiac and thoracic surgery. The CT surgery service provides advanced surgical treatments, including coronary artery bypass grafting, aortic and mitral valve replacement, extracorporeal membrane oxygenation, and heart transplantation. We value individuals who are professional, with strong interpersonal and communication skills, and able to prioritize and adapt to changing situations. Our CT surgery APPs manage patients collaboratively with CT Surgery Fellows and Physicians, perform bedside procedures, and assist in the operating room. The CT Surgery APP assists in the development of practice protocols, evaluation of quality of care, and education of patients, families, and other health care practitioners. Responsibilities 90% Inpatient - Direct Patient Care (may include some or all of the responsibilities listed) 1.1.Independently (independent APP encounter) or collaboratively (shared physician and APP encounter) evaluates and manages patients. Includes taking histories, performing physical examinations, responding to changes in patient status, performing consults, ordering or furnishing medications/diagnostic tests/treatments/therapies/consultations/referrals/durable medical equipment (DME)/home health services.
