Resident Care Coordination:

  • Overseeing and coordinating comprehensive resident care plans, ensuring individualized and holistic approaches.

  • Conducting thorough resident assessments and collaborating with residents, families, and interdisciplinary teams to optimize well-being.

  • Monitoring resident health status, identifying changes, and implementing appropriate interventions.

  • Serving as a primary point of contact for residents and their families regarding care concerns and needs.

  • Maintaining accurate and up-to-date resident records in compliance with all regulations.

Clinical Nurse Education:

  • Developing, implementing, and evaluating engaging educational programs and in-services for nursing staff.

  • Providing hands-on training and mentorship to enhance clinical skills and promote best practices.

  • Staying current with the latest advancements in senior care and translating this knowledge into practical training.

  • Assessing the educational needs of staff and creating tailored learning opportunities.

  • Ensuring compliance with all regulatory and company-specific training requirements.

What You'll Bring:

  • Active Registered Nurse (RN) license or Licensed Practical Nurse (LPN) in the state of Indiana (required).

  • Minimum of 2 years of experience in long-term care, skilled nursing, or assisted living, with a strong emphasis on senior care.
