Reporting Directly to the Hospital at Home Director of Nursing, The Hospital at Home Staff RN is responsible for providing high-quality patient care to various patient populations within the framework of the BILH Hospital at Home program.  The program provides inpatient-level care for patients in the comfort of their home. The Hospital at Home Staff RN is a Field Based Nurse with some virtual nursing in a centralized command center located in Charlestown. This is a fully on site position.  Field assignments can be within a 25 mile radius of any BILH hospital. 

Job Description:

Essential Duties & Responsibilities including but not limited to:

•    Utilizing monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home
•    Navigating patients and families across the continuum of care
•    Monitoring and responding to biometric data per Hospital at Home Command Center protocols
•    Conducting audio/visual patient care visits as directed by the care team and/or protocol
•    Ensuring all interventions in the patient’s care plan are implemented efficiently with a patient centered approach
•    Leveraging the resources and expertise of supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundation
•    Adhering to the clinical protocols and be willing to obtain the necessary training to provide care within the context of the providing evidence-based care
•    Promoting professional practice and a culture of safety; willingness to engage in process improvement efforts


Clinical Roles and Responsibilities:

•    Providing protocol driven assessment, care coordination and interventions as appropriate informed by Hospital at Home protocols for diverse patient populations with a focus on oncology, medicine and post- operative care
•    Exhibiting sound clinical decision making and critical thinking skills
•    Maintain and practice in accordance with nursing guidelines per state and hospital regulations
•    Facilitating admissions to l Hospital at Home utilizing Hospital at Home guidelines
•    Serving as a role model for professional nursing practice
•    Utilizing nursing knowledge to identify, prevent and/or solve complex acute and/or recurring patient care problems
•    Sharing expertise with the multidisciplinary team and serve as a resource to staff in area(s) of expertise
•    Supporting research efforts to advance knowledge and promote evidence-based practice
•    Preforming other duties and responsibilities as assigned by the Senior Director of Nursing

Quality and Safety Responsibilities:

•    Ensure compliance with reporting requirements for Sentinel Events and Serious Incidents
•    Participate in quality and safety investigations and root cause analyses
•    Take appropriate action to ensure patient safety as a result of any investigation; and
•    Serve as a resource in quality improvement processes and technique

Minimum Qualifications:

Education:  
•     Nursing Diploma or ASN, BSN preferred

Licensure, Certification, Registration: 
•    A current Massachusetts RN license
•    BLS certification

Pay Range:

$37.00 - $95.74

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.  Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.

Responsible for the planning, delivery and management of direct patient care utilizing the nursing process and adhering to the standards of nursing practice. I. Major Responsibilities: 1. Utilizes the Nursing Process for the delivery and management of patient’s care from Admission to Discharge, appropriate to the developmental age of the patient population. 2. Performs patient assessment/reassessment according to standard procedure. 3. Develops and coordinates individual plan of care including discharge planning. 4. Directs, supervises, coordinates and evaluate nursing care within assigned clinical setting. 5. Delegates appropriately to others in accordance with job description and competency level. II. Position Qualifications: License/Certification/Education: Required: 1. Current Massachusetts Nursing licensure and registration. 2. BLS is a requirement for every RN unless advance cardiac life support (ACLS) certification is current, in addition to any unit specific certifications and competencies. Preferred : 1. Bachelor of Science degree in Nursing highly preferred. Experience/Skills: Required: 1. Thorough knowledge of the discipline of Nursing. Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements. Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents. III. Physical Demands and Environmental Conditions: Work is considered medium. Position requires work indoors in a patient care environment.
