Job Overview

JOB DESCRIPTION

This position focuses on promotion, protection and restoration of workers' health within the context of a safe and healthy work environment, collaborating with workers, employers and other professionals to: prioritize interventions; develop and implement interventions and programs; evaluate care and service delivery; and maximize productivity and wellness of workers within client companies. The foundation for this position is research-based with an emphasis on optimizing health, preventing illness and injury and reducing health hazards. Depending on client company needs, the incumbent may focus/specialize in one or more of the following areas: disability case management, disease/condition management, injury prevention, first aid/emergency preparedness, and/or health education and promotion.

Job Requirements

Associate's Degree or Diploma in Nursing

Basic Life Support for Healthcare Providers (BLS)

Registered Nurse

Current Ohio licensure

Occupational health, wellness, prevention, or condition management experience

Computer literacy

Understanding of ICD9/ ICD10/ CPT 4 codes

Experience with OSHA/ ADA/ Workers Compensation Excellent communication and interpersonal skills

3-4 years experience Professional in Nursing

Job Responsibilities

Safety/Health Promotion and Demonstrating Value/ROI - Collaborates with client company management and employees to establish/maintain a workplace that will help to protect workers from occupational illness and injury and maintain regulatory compliance. Follows and implements SHARE processes that monitor, track, trend, and report outcomes so that value and ROI are clearly demonstrated and reported to the customer on a quarterly basis.

Manages program adminisration by utilizing and including the customer in the SHARE process of: goal setting, planning, implementation, evaluation and measurement as a means of accomplishing the desired occupational health goals for the program. May provide oversight and direction to LPN's and wellness coordinators. May function as a leader/member of Corporate Health team of professionals where an integrated service model is in place.

Develops and operates the occupational health program according to the established guidelines and regulations set forth by the federal, state and local laws, professional organization and the SHARE Occupational Health Program. Completes all requested/required reports timely and attends all required/requested meetings/presentations.

Provides Health Education/Screening/Counseling through Establishing/maintaining a system for health/wellness promotion, screening, education, and counseling as well as disease management and health coaching.

Ensures First Aid/Emergency Preparedness by applying nursing skills following SHARE Medical/Nursing Protocols and SHARE Clinical Procedures to provide a workplace program for the timely and effective treatment of first aid and emergency incidents.

Provides pre and post absence and disability risk assessment to develop targeted plans for pre-identified conditions (mental health/ depression, cardiovascular, and diabetes or metabolic syndrome). Performs interventions that focus on impacting behavior change and maximizing self-management to positively impact the health of employees with chronic conditions. Collaborates with appropriate departments to facilitate employee success in utilizing available resources (i.e. HR, Workers Comp, Benefits, EAP, Health/Wellness) to improve health status.

Provides passive case management oversight to certain cases which involves review, monitoring, and support of work-related and personal disability claim supplier and third party administration activities. Provides active case management interventions to appropriate cases and assist with transitional work program and early return to work program management as well as coordination with long term disability suppliers to document continued disability.

The position also collaborates, in a collegial manner, with physicians and other health care team members in meeting the patient's needs. This position may delegate certain aspects of care to those under their direct supervision, within the scope of nursing practice Job Responsibilities Performs initial and ongoing assessment of patient and family. Completes initial assessment tool. Documents ongoing assessment per unit/TriHealth guidelines. Communicates assessment findings to other health care providers as appropriate. Includes health counseling and health teaching needs in assessment. Plans care for patient and family based on assessment, standards of care, and optimal specific outcomes. Initiates and individualizes appropriate patient care guidelines/plan of care or clinical pathways. Updates current plan of care as needed based on patient/family input and healthcare needs. Develops both short and long term goals with patient/family and healthcare team including discharge planning. Communicates plan of care to others. Provides a safe, therapeutic environment, maintains patient’s autonomy, dignity, and rights, and is sensitive to patient diversity. Seeks resources to help form ethical decisions. Balances patient's needs and those of the unit/facility. Recognizes emergency situations and takes appropriate action. Completes patient assignment including documentation within scheduled timeframe. Bases interventions on clinical data and desired outcomes and documents accordingly. Trains/educates other staff and acts an expert resource in specialty area through abilities in existing and newer knowledge and skills. Effectively communicates information relative to surgical cases. Carries out physician orders. Evaluates the plan of care for patient based on optimal specific patient outcomes. Documents the patient/family response to care including teaching. Collaborates with the patient/family and with other members of the health care team, including physicians, to revise plan of care as needed. Supervises the care that was delegated to other health care team members. Performs technical skills according to policy and procedure and accepted standards within their area of practice. Safely administers medications/treatments and monitors their effects. Uses all equipment in a safe, appropriate manner. Assumes organizational and educational responsibilities. Identifies areas for professional and self-improvement through self-assessment and peer review, as well as the continuing education of team members continuing education, formal education, professional organization membership, and/or certification. Shares knowledge and skills with colleagues and others. Provides peers with constructive feedback regarding skills and knowledge.
