Certified Home Health Aide Trainee, VNA of Cape Cod

Purpose of Position: Under the direction of the Paraprofessional Team Leader the Certified Home Health Aide (CHHA) performs assigned duties in the patient’s home in a safe and considerate manner as prescribed by the case manager.

Description

  • Maintains open communication with case manager and/or Team Leader/Assistant Team Leader regarding health, safety and environment status of patients. Reports all changes in a timely manner.
  • Complies with State/Federal regulatory acts.
  • Perform all skills with competency, as indicated on the skills checklist.
  • Communicates with Clinical Coordinator regarding all scheduling concerns relating to patients which may include, but not limited to changes such as times, days, patient absences and discharges.
  • Communicates with case manager, and if not available, the Team Leader/Assistant Team Leader regarding any case concerns, patient problems, planned time off or other general schedule changes.
  • Completes and submits required documentation, i.e., patient and employee time sheets, etc., accurately and on a timely basis.
  • Report all concerns of possible patient abuse or neglect to the case manager and/or the Team Leader.
  • Establishes and maintains a therapeutic relationship with patients.
  • Perform only those tasks as prescribed by the case manager on the home health aide plan of care.
  • Encourage patient in achieving an optimum level of independence by encouraging his/her participation in personal care and in interests and activities in the home, family and community.
  • Provides or assists with post mortem care after pronouncement.
  • Perform additional tasks as may be assigned by the Team Leader and/or case manager, depending on the specific situation or on the Home Health Aide’s experience and capabilities. Example: routine uncomplicated colostomy case and decubitus care.
  • Provides understanding and emotional support to patient and significant others.
  • If eligible, may participate in the Patient Transportation program for the Private Services department.
  • All other duties as assigned

The Home Health Aide Trainee Program:

  • Includes classroom curriculum with an accredited Home Health Aide Program.
  • Includes on-the-job training.
  • Earn while you learn.
  • Must successfully complete the program with Certification within 3 months.

Qualifications

  • Possess and maintain certification as a Home Health Aide or equivalent as approved by the Agency.
  • One year satisfactory employment history preferred. Experience in care of the elderly preferred.
  • Satisfactory completion of Skill Assessment Test. Able to record messages and keep records.

HOME HEALTH TRAINEE EDUCATION/EXPERIENCE/TRAINING:

  • High School Diploma or equivalent preferred.
  • Ability to read, write and communicate in English.
  • Minimum six months of satisfactory employment history.
  • Satisfactory completion of Skill Assessment Test.
  • Able to understand and carry out direction and instruction.
  • Ability to operate a Smart Phone Based Application
  • Upon completion of the Certified Home Health Aide Program, incumbent will transfer into the Home Health Aide role in the previously agreed upon assigned area of coverage.

The Home Health Aide Trainee Program:

  • Includes classroom curriculum with an accredited Home Health Aide Program.
  • Includes on-the-job training.
  • Earn while you learn.

Schedule Details: 30 Hours Per Week, Monday–Friday, 8a–4p, Every Other Weekend, Occasional Holidays. Area: Lower/Outer Cape Organization : Visiting Nurse Association

32 hours - with every other weekend rotation - Must have current CT CNA registration JOB TITLE: Home Health Aide DEPARTMENT: Home Care & Hospice ACCOUNTABILITY: Home Health Aide Scheduling Supervisor Clinical direction from Home Health Aide Field Supervisor WORK AREAS: Client Homes Setting CLASSIFICATION: Non-exempt POSITION SUMMARY: The Home Health Aide follows the plan of care as directed by the Case Manager (Nurse and/or therapist). Provides personal care and related homemaking duties for the adult client with short-term rehabilitation, and care of the sick during terminal stage of life. The Home Health Aide is responsible for communication of the implementation of the plan of care and reports changes in the clients’ status. The client’s home setting may be a private residence, assisted living or skilled nursing facility. The Home Health Aide consistently follows the McLean mission and code of conduct. JOB COMPETENCIES: 1. Follows the care plan carefully providing clients with personal care as needed, i.e. Provides personal care activities while encouraging client participation with ADL’s such as: Bathing (bed bath, tub or shower) Grooming (shampoo, shave, common nail care as determined by RN) Mouth care Skin care (as determined by RN) Toileting (commode, toilet or bed pan) Dressing Feeding (prepares varied and nutritious meals, serves and feeds as necessary) Provides assistance to increase the level of independence or maintenance of function according to plan of care Transfers (from bed to chair, etc.) using gait belt or mechanical lift according to care plan Exercises Ambulation with or without assistive device Assist client with medication reminders Performs other RN-assigned client tasks within the scope of the job description. Participates in twelve hours of agency-approved education annual, six of which are related to hospice care. Communicates with supervisor to monitor progress in order to meet annual requirement. Submits accurate and timely documentation of the care provided during the visits, travel time and mileage through telephonic system. Submits completed paper documentation only when necessary. Performs other duties as assigned. WORKING CONDITIONS: Typical Physical Demands : Requires full range of body motion including position, assisting and transferring clients using minimal, moderate, or maximal assist. Requires manual dexterity and hand eye coordination. Requires the ability to walk up and down stairs. Requires the ability to drive a motor vehicle. Requires vision and hearing within normal range. Requires the ability to read and write. Requires occasional lifting of greater than 50 pounds, often combined with bending and turning and the ability to push client in a wheelchair, both in the home and outside on paved surface. Contact with clients, families and staff. Attendance at in-services at McLean or other locations is required. Some potential exposure to communicable diseases.
