Position Overview:

The CNA is a member of the community nursing team whose responsibility is to assist professional nursing personnel by delivering direct hands-on nursing care to ambulatory and non-ambulatory residents on a daily basis. The CNA is also responsible for assisting the residents to achieve the highest level of functioning while living as normal a lifestyle as possible.

Key Responsibilities:

• To perform or assist the resident with completing Activities of Daily Living (ADL).

• Respond to residents call lights to provide maximum comfort, safety and privacy.

• Transport residents by ambulation or by wheelchair to areas of community (dining room, lobby and activities).

• Report any significant findings/observations to nurse immediately.

• Assist residents on admission, transfer and discharge to make a smooth transition.

• Responsible for the safekeeping of residents’ personal possessions.

• Keep resident rooms neat and safe.

• Follow proper infection control techniques

• Respect the rights of each resident, particularly privacy and confidentiality.

• Re-teach resident to perform ADL skills.

• Provide restorative care to prevent pressure sores, contractures, and complications.

• Participate in Continuous Quality Improvement and ensures follow-up occurs.

• Converse with resident and family; assist them on adjustment to the community.

• Ensure adequate hydration and nutrition of resident; report deviations from norm to the Charge Nurse promptly.

• Perform blood pressure, pulse, respirations, intake/output and weights accurately.

Qualifications:

• Certified and state tested as a Certified Nursing Assistant in the state employed; certificate must be active, valid, and in good standing.

Activity Aide

Part time Evenings and Weekends The Activity Aide assists the Activity Director in carrying out a planned activity program for residents. Maintains records, transports residents to and from activities. Performs varied clerical and department maintenance functions. The person holding this position is delegated the responsibility for carrying out the assigned duties and responsibilities in accordance with current existing federal and state regulations and established company policies and procedures. *CNA preferred but not required. Job Responsibilities • Aides assist the recreational activities leader or activities director, helping patients or participants use equipment and supplies. Activities may include arts and crafts, outdoor activities, sports, music or drama. • Prepares equipment and ensures the safety of patients or participants in the activity. • Assist in leading the participants in group activities, such as an exercise class or art project. • Proper record keeping of equipment and its use. • Assist in planning programs for residents, which can help to provide socialization, relaxation and exercise. Education/Experience: • Must possess a high school diploma • Must possess a valid driver's license with F-Endorsement • Must be able to drive the facility van • Must be able to work flexible hours including evenings, weekends and/or holidays • Must be able to work independently and as a member of a team • Must present a positive image of the facility • Must meet health assessment requirements, including two stage Mantoux skin tests and one-step annually thereafter Additional Skills Activity aides should be energetic and personable to work with residents or participants.
