We’re seeking an experienced and visionary LPN Clinical Coordinator to join our leadership team and help drive clinical excellence, grow our census, and foster relationships within the healthcare community.
Position Overview
The LPN Clinical Coordinator supports the delivery of quality patient care by balancing administrative responsibilities with occasional fieldwork. This role serves as a liaison between patients, families and caregivers while ensuring compliance with New York State Department of Health regulations. The Clinical Coordinator is responsible for maintaining accurate documentation, timely communication and consistent coordination of patients needs including but not limited to coordinating with providers, supporting caregivers, assisting with audits and reinforcing agency policies to promote safe high-quality, compassionate care.
Qualifications:

Current Licensed Practical Nurse (LPN) license in good standing

Minimum (2) two to (3) three years of verifiable clinical experience in homecare, long-term care or similar setting preferred

Prior experience in a supervisory or care coordination role strongly preferred

Willing to meet with and speak with patients, their family, and their medical professionals to develop, update and maintain proper care plans.

Experience with electronic health records (EHR) and care management systems is preferred.

Able to manage both field and office responsibilities

Strong communication and PC skill
Responsibilities
-
Performs or supervises case management activities for the individual patients, including but not limited to, assessments, patient/family teaching and development and updating of the care plan or paraprofessional task sheet under RN supervision.

Assist with intake of new patients including but not limited to confirming demographics, emergency contacts and provider information

Coordinates with home office staff to ensure the appropriate assignment of personnel to meet the specific need of an individual client.

Answer incoming phone calls from prospective clients and perform initial care assessments of incoming client as needed under RN supervision

Build and maintain relationships with clients, community agencies, and professional organizations.

Audit and review patient charts for compliance and obtain the necessary physician orders for the home care cases as required by State regulations.

Ensures that changes in the patient’s medical or mental condition are reported to the attending physician and to Agency employees involved in the patient’s care.

Coordinates arrangements with other health care and equipment providers to assure continuity of care and adherence to the patient’s care plan.

Maintains compliance to all agency clinical and administrative policies, procedures and regulatory requirements.

Assist with evaluating and supervising all professional and ancillary homecare staff for conformity with Agency policies and procedures, professional standards of home health care, state health statutes and home care requirements.

Maintains clinical records in accordance with Agency policy as well as state and federal regulations.

Participates in quality improvement activities and evaluations of the Agency.
