Responsibilities: • Travel to client homes across assigned counties • Complete nursing assessments and reassessments as needed • Develop, review, and update client care plans • Monitor client condition and report changes as appropriate • Provide support, guidance, and supervision to caregivers • Ensure care is being provided according to the client's needs and care plan • Communicate with clients, families, caregivers, and office staff • Maintain accurate and timely documentation • Help ensure compliance with company policies and home care requirements • Promote a compassionate, respectful, and professional care environment Requirements: • Active RN license • Prior home care experience preferred • Willingness to travel to client homes in multiple counties • Valid driver's license and reliable transportation • Strong assessment, communication, and documentation skills • Ability to work independently and manage time effectively • Professional, compassionate, and reliable • Must meet all company hiring and onboarding requirements You're not a patient, you're family.
