Job Responsibilities:

-Manage all aspects of patient care from initial admission assessment to weekly visits, to discharge when goals are met.

-Coordinate patient care with their physician as well as other disciplines involved in care (PT,OT,ST,HHA)

-Upon admission, develop plan of care tailored to patient’s individual needs, including establishing goals and discharge planning.

-Revise the plan of care as necessary, depending on patient status and any changes.

-Communicate with physician regularly on patient condition and to request any orders necessary.

-Provide direct nursing care and education based on the care plan.

-Completes all assessments and nursing notes timely and thoroughly.

-Collaborate with other home health disciplines for the best patient outcomes, which includes attending case conference meetings regarding patient overall care.

-Supervision of HHA and LPN visits, when applicable.

-Communicate effectively with other team members, including administrative supervisors regarding scheduling, patient updates, or any other daily updates.

-Participate in on-call rotation when applicable.

 

Qualifications:

Graduate of an accredited school of nursing with Associate/Bachelor’s degree.

Current unencumbered RN license and CPR certification

1 year of home health experience required

1-2 years of acute care RN experience required.

Responsibilities: RN (Registered Nurse) Complete daily electronic forms for each assigned patient within a secure Electronic Medical Record (EMR) System. Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients. Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager. Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation. Report patient care, conditions, and progress to the patient's physician, pharmacy, and Clinical Manager continuously. Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems. Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation. Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care.
