Job Responsibilities / Duties:

  • Promotes resident hygiene and comfort.

·         Assist residents Bathing, dressing, grooming, toileting, walking, and other personal care needs

  • Distributes food/water and feeds residents.

  • Assists with repositioning and transferring residents.

  • Ambulates and transports residents.

  • Records and totals fluid intake and output.

  • Weighs residents.

  • Documents on flow sheets.

  • Assists with admission, discharge and postmortem care.

  • Responds to call lights in a timely manner.

  • Performs perineal care and uncomplicated colostomy care.

  • Reports all observations & assessments to the Team Leader/Nurse Manager/Charge Nurse.

  • Takes and records vital signs.

  • Makes beds and keeps resident’s units clean and in order.

  • Prepares room for housekeeping after resident discharge.

Skill / Experience Requirements.

·         Current (insert State) CNA Certification.

·         Previous Long Term Care experience a plus.
