Park Gardens Rehabilitation & Nursing Center, a skilled nursing facility, located in Bronx County, New York, is actively looking to hire a clinically superb full time LPN Charge Nurse. The LPN Charge Nurse will manage an assigned caseload to assure the quality of services provided meet the high standards of professionalism and those required by City, State and Federal Regulations.

Job Duties

Oversee and coordinate nursing care for assigned patients.
Guide and support field nurses; serve as a clinical resource.
Review clinical documentation for accuracy, completeness, and compliance.
Communicate with patients/families, physicians, insurance case managers, and internal teams to advance safe, timely care.
Participate in quality improvement and adhere to agency policies/standards.

Requirements

Qualifications
Valid NY LPN license in good standing.
Associate’s degree required; BSN preferred.
2+ years nursing home experience (minimum).
Strong communication and computer skills

The Clinical Supervisor I is a licensed health care professional or clinician who oversees and mentors less experienced clinical staff, assigns work, maintains schedules and assists the Practice Manager and other leadership in the implementation of policies, workflows and processes. Responsibilities Oversees clinical staff in assigned area and assists department management in implementation of day-to-day activities and processes. Responsible for training staff on clinical office protocols and techniques. Responsible for documentation and maintaining patient immunization records, Coumadin registry, administering vaccines/injectables and other related clinical duties.
